How to Check Your Heart Attack Risk at Home

You can't feel a heart attack building — but you can measure the risk. Here are the blood markers that actually predict cardiovascular risk, and how to check the key ones from home.

Heart disease is the leading cause of death for American adults, and its reputation as a “silent” killer is well earned: the first symptom is often the heart attack itself. By the time you feel something, plaque has usually been building in your arteries for years or decades.

The good news is that the process leaves a trail long before it becomes an emergency — in your blood. The markers that predict cardiovascular risk are measurable, they move in response to what you do, and you no longer need a clinic appointment to check the most important ones. Here’s what actually drives heart attack risk, and how to check it from home.

First, an important distinction

“Checking your heart attack risk at home” means measuring your long-term risk — the biology that determines how likely you are to have a cardiovascular event over the coming years. That’s what blood markers reveal, and it’s what this article is about.

It is not the same as detecting a heart attack that’s happening right now. No at-home blood kit can do that. If you’re having chest pain, pressure, shortness of breath, pain spreading to your arm or jaw, or you otherwise think you might be having a heart attack, call 911 — don’t order a test. At-home risk testing is a tool for prevention, done when you feel fine, so you can act years before an emergency is on the table.

The numbers that actually predict risk

For decades, “get your cholesterol checked” meant looking at total cholesterol and maybe HDL. That picture is now considered incomplete. Here are the markers that carry the most predictive weight for cardiovascular risk.

ApoB — the marker most researchers now trust most

Every artery-clogging particle in your blood — LDL, VLDL, and others — carries exactly one apolipoprotein B (ApoB) molecule. So ApoB is essentially a direct count of the particles that drive atherosclerosis. Two people can have identical LDL cholesterol numbers but very different ApoB, and it’s the ApoB count that tracks better with actual risk. Many lipidologists now consider it the single best routine blood marker for cardiovascular risk. If you check one “advanced” number, this is it.

LDL and non-HDL cholesterol

LDL cholesterol is still a cornerstone — it’s the cholesterol carried by those artery-damaging particles, and lowering it is one of the most consistently proven ways to reduce heart attack risk. Non-HDL cholesterol (total cholesterol minus HDL) captures all the “bad” cholesterol in one number and is a stronger predictor than LDL alone.

Triglycerides and the triglyceride-to-HDL ratio

High triglycerides signal a metabolism under strain and travel with the small, dense particles that are especially good at lodging in artery walls. The triglyceride-to-HDL ratio is a useful, quick read on that pattern and on insulin resistance — a major, often-missed driver of heart risk.

hsCRP — the inflammation piece

Atherosclerosis isn’t just a plumbing problem; it’s an inflammatory one. High-sensitivity C-reactive protein (hsCRP) measures low-grade inflammation in the body, and an elevated level flags risk that a lipid panel alone can miss. Two people with the same cholesterol can have very different risk if one is running hot with inflammation.

Lipoprotein(a) — the genetic wild card

Lipoprotein(a), or Lp(a), is a largely inherited particle that raises heart attack and stroke risk independently of everything above. It’s mostly set by your genes and stays fairly stable through life, so it only needs to be measured once — but roughly one in five people carry a high level and have no idea. It’s worth knowing your number at least one time. (Lp(a) isn’t part of the at-home kit below — it’s best added to a full lab panel, more on that in a moment.)

What you can check from home

Two things worth measuring that aren’t in the kit

A complete risk picture includes a couple of things a blood collector can’t cover on its own:

  • Blood pressure. High blood pressure is one of the biggest and most modifiable heart-attack risk factors, and it’s genuinely easy to check at home — a $30–$50 upper-arm cuff, used a few times a week, tells you far more than a single reading at the doctor. This is the most valuable at-home heart measurement that isn’t a blood test.
