What Causes Easy Bruising, and What Blood Tests Can Check

Some causes of easy bruising can be checked with bloodwork and a large share cannot. Here is the short list of patterns that need prompt medical attention, the causes that are answered by your skin and your medicine cabinet rather than a lab, and the exact test to start with for each blood-testable contributor.

Some causes of easy bruising can be checked with a blood test, and a large share cannot. The checkable list is short and specific: platelet count, how quickly your blood clots, von Willebrand factor, liver function, vitamin C, and vitamin K. The uncheckable share is bigger than most people expect, because thinning sun-exposed skin, a medicine or supplement you already take, and ordinary capillary fragility are answered by your history and an examination rather than by a lab. Separately from both, a small number of bruising patterns need medical attention promptly rather than a test you order yourself.

This page sorts those three groups. It starts with the patterns that should send you to a clinician, says plainly which common causes bloodwork cannot see, then names the marker to start with for each blood-testable contributor and where that marker stops being able to tell you anything.

When easy bruising needs medical attention rather than a lab order

Some bruising warrants a clinical evaluation soon rather than a self-ordered test. Contact a clinician promptly if bruising arrives with any of the following:

  • bleeding from the gums, or frequent or prolonged nosebleeds, alongside the bruising
  • petechiae, meaning pinpoint flat red or purple spots that do not fade when you press on them, particularly in crops on the lower legs or inside the mouth
  • bruises that are unusually large, painful, or raised, or that keep spreading after they appear
  • bruising alongside fever, night sweats, unexplained weight loss, or new persistent fatigue
  • bruises on the trunk, back, or abdomen with no injury you can recall

Every item on that list is something happening alongside the bruising, and that is the point. Bruising on shins, forearms, and the backs of hands, in someone who is otherwise well and whose bruises heal normally, is a different situation from bruising that comes with mucosal bleeding, petechiae, systemic symptoms, or bleeding at several sites at once. Bleeding from more than one site, mucosal bleeding, bleeding out of proportion to the injury, and a family history of bleeding are the features that move an evaluation from watchful to prompt [1][20].

This matters because the conditions behind that second pattern include immune thrombocytopenia [9], liver disease [12], an inherited bleeding disorder [11], and, less commonly, a blood cancer such as acute leukemia, whose recognized presenting features include bleeding, fever, infection, and feeling weak or tired [19]. None of those is diagnosed from a symptom list, and none of them is the likely explanation for most easy bruising. What the pattern does is decide who needs to be seen and how quickly. The right response to it is an appointment, not a wider panel of tests.

Two situations need urgent care rather than an appointment: bleeding that will not stop after a cut or a dental procedure, and significant bruising or swelling after a head injury.

A large share of easy bruising is not a blood-test question

Before the testable list, the honest statement. Much of easy bruising is skin, medication, or ordinary capillary fragility, and none of that is answered by bloodwork.

Photoaged skin. Actinic purpura, also called senile purpura, is the flat purple patch that appears on sun-exposed forearms and the backs of the hands in older adults after minimal or unremembered trauma. It reflects chronic ultraviolet damage to the connective tissue supporting small blood vessels, so those vessels tear with contact that younger skin would absorb [2]. It is common, it is recognized on sight, and there is no blood test for it.

Medications. A medication review answers more easy-bruising questions than a lab panel does [1]. The usual contributors are corticosteroids, both systemic and potent topical ones, which thin the skin and cause easy bruising with longer use [3]; anticoagulants including warfarin and the direct oral anticoagulants; antiplatelet drugs including aspirin and clopidogrel; and nonsteroidal anti-inflammatory drugs. For warfarin specifically, PT/INR is the test used to monitor its effect, and the dose that follows from a result is a conversation with the clinician who prescribes it rather than something to act on from a self-ordered test [4].

Supplements, with the evidence stated honestly. Fish oil, ginkgo, and high-dose vitamin E are the supplements most often raised in this context, and they are worth telling your clinician about. The evidence that they actually cause bleeding is weaker than their reputation. A systematic review of standardized ginkgo biloba extract found no significant effect on bleeding risk [5], and a large analysis of cardiac surgery patients found no increase in perioperative bleeding with fish oil [6]. High-dose vitamin E is the one with a clearer caution, because it can interfere with vitamin K dependent clotting and is specifically flagged in people taking anticoagulants [7].

Inherited connective-tissue differences. Some people bruise easily because of how their connective tissue is built. The Ehlers-Danlos syndromes are the recognized group here, and easy bruising is a feature of several of their types [8]. They are identified by clinical criteria, examination, and in some types genetic testing, not by a routine blood panel [8].

Ordinary capillary fragility. Some people simply bruise more readily than others, at every age, with no identifiable disorder and a completely normal workup.

Two consequences follow, and they matter for how you read any result:

  • Normal bloodwork does not mean nothing is wrong. The most common explanations for easy bruising would produce an entirely normal panel, and a normal panel does not rule out a serious cause in someone whose pattern of bleeding is concerning.
  • An abnormal marker does not prove it caused the bruising. It identifies a contributor worth investigating, which is a different and weaker claim.

What blood work can and cannot check for easy bruising

Blood testing does not diagnose easy bruising. It checks a specific set of conditions that make bleeding into the skin more likely, and it narrows the list rather than closing it.

Blood work can check your platelet count and the rest of a blood count, how quickly your blood clots through each of the two arms of the clotting cascade (prothrombin time covers one, activated partial thromboplastin time covers the other), fibrinogen, von Willebrand factor, liver injury and the liver’s synthetic function, vitamin C, vitamin K, and iron stores.

Blood work cannot check how fragile your capillaries are, how thin or sun-damaged your skin is, whether a medicine or supplement you take is the reason, whether your platelets work properly even when there are enough of them, or whether an inherited connective-tissue difference explains it. Those are answered by history, examination, and a medication review.

The blood-testable contributors, and the marker to start with for each

Most of the entries below are real, checkable causes of bleeding into the skin, and each has one sensible place to start. The last one is deliberately different, and it is labelled as such: iron deficiency is not a cause of easy bruising, and ferritin earns its place here as a downstream consequence worth checking when bruising comes with bleeding from somewhere else.

One caveat applies to all of them. A single marker is a screen, not a verdict. A normal result narrows the list, but for several of these a normal result does not by itself rule the cause out, either because the test measures one part of a system with several parts or because the value moves with things that have nothing to do with bruising. Each section below says where its own marker stops.

Low platelets: check a complete blood count

Platelets form the first plug when a small vessel is injured, so when there are too few of them, minor knocks bleed into the skin more than they should. Low platelets have several routes, and immune thrombocytopenia, in which the immune system destroys platelets, is one of the recognized ones [9]. A complete blood count with differential and platelet count is the first test, and the sensible first test for unexplained easy bruising in general, because the same draw also shows the white-cell and red-cell picture that matters for the red-flag patterns above.

Where a platelet count stops: it counts platelets, it does not test whether they work. Inherited and acquired platelet function disorders exist and are identified with dedicated platelet function studies that a normal count gives no information about [10]. A normal platelet count also says nothing about a clotting-factor problem, which is a separate system with its own tests. So a normal count narrows the question rather than answering it.

Start with a complete blood count (CBC) with differential and platelet count. If you already know the count is the specific question, the narrower platelet count blood test measures it on its own. Clotting time: check PT/INR, and aPTT alongside it

The clotting cascade has two arms, and the two standard screening tests cover one each. Prothrombin time, reported with the international normalized ratio, covers one of them. It is the standard test for monitoring warfarin, and it reflects the clotting factors your liver makes and the ones that depend on vitamin K [4]. Activated partial thromboplastin time, written aPTT and often labelled simply PTT on a lab menu, covers the other arm. The pair is meant to be read together, and the combinations are what carry the meaning: a normal PT with a prolonged aPTT indicates a disorder of the intrinsic pathway, and a prolonged PT with a normal aPTT points to the extrinsic side [20].

Where PT/INR alone stops, and this is the reason not to order it by itself for a bleeding history: it reflects one arm. An intrinsic-pathway problem can sit behind a completely normal PT/INR and a completely normal platelet count, and aPTT is the screen that catches it [20]. Even the pair has a limit worth knowing before you read too much into a clean result. A normal PT with a normal aPTT does not close the question, because that combination itself points toward a platelet disorder, and the commonest condition sitting there is von Willebrand disease [10][11][20].

A prothrombin time with INR (PT) blood test is the right order when PT itself is the specific question, for instance alongside a liver result. If you are investigating a bleeding history rather than one number, the combined PT with INR and PTT test covers both arms in a single draw. And if that bleeding history is convincing, this is the point to bring in a clinician rather than to keep adding single tests. The current primary-care recommendation for initial laboratory testing is a complete blood count, a peripheral blood smear, PT with INR, aPTT, and fibrinogen, read together, with hematology consultation when that evaluation indicates a bleeding disorder or when suspicion stays high despite a normal workup [20]. If you take warfarin, your monitoring schedule and any dose change belong with your prescribing clinician [4].

Inherited bleeding disorder: check von Willebrand factor antigen

Von Willebrand disease is the most common inherited bleeding disorder, and its usual presentation is mucocutaneous: easy bruising, nosebleeds, heavy menstrual bleeding, and bleeding after dental work or surgery [11]. If you have bruised easily for as long as you can remember, or other people in your family do, this is the contributor that a general panel will miss.

Where the antigen stops, and this one has real limits worth understanding before you order it. Von Willebrand factor antigen measures how much of the protein is present. A complete initial workup also measures how well it works, through a von Willebrand factor activity assay, and measures factor VIII [11]. Levels are also genuinely variable: they are lower on average in people with blood group O, and they rise with stress, exercise, inflammation, pregnancy, and estrogen [11]. A single normal antigen result therefore does not exclude the condition, and repeat testing is often needed before the question is settled [11].

Because of those limits, this is the one contributor on this page where a single self-ordered marker is not the right opening move for a strong suspicion. If you have a convincing lifelong bleeding history, the initial workup to ask a clinician for is all three together: von Willebrand factor antigen, platelet-dependent von Willebrand factor activity, and factor VIII [11]. Ordering a von Willebrand factor antigen (vWF) blood test on its own gives you one of those three. That is informative when it comes back low, and it is not a clearance when it comes back normal.

Liver: check a hepatic function panel

The liver makes most of the clotting factors, so advanced liver disease contributes to bruising through two routes at once: reduced clotting-factor synthesis, and a low platelet count from an enlarged spleen and reduced thrombopoietin [12]. Bruising is rarely the first sign, but it is a recognized part of the picture.

Where a hepatic function panel stops: it shows liver injury through the enzymes, and synthetic function through albumin and bilirubin. It does not itself measure your blood’s clotting capacity. PT/INR and a complete blood count complete that picture, and even then there is a subtlety worth knowing. In chronic liver disease both the clotting factors and the natural anticoagulants fall together, so PT/INR reflects only one side of a rebalanced system and is a poor predictor of bleeding risk in that specific setting [12]. This is a case where the number needs a clinician reading it in context.

Vitamin C: check vitamin C

Scurvy is uncommon, but it is real, it still occurs, and it is reversible. Its skin presentation is distinctive: bruising, bleeding around hair follicles, corkscrew hairs, and swollen bleeding gums [13][14]. It develops in people whose diets have carried very little fruit and vegetable intake for months, which happens more often than the word “scurvy” suggests, particularly alongside restricted eating, alcohol use disorder, or isolation.

Where a vitamin C result stops: plasma ascorbic acid reflects recent intake rather than long-term status, and plasma concentrations fall during infection, inflammation, and acute illness [15]. A result drawn after a week of eating differently, or in the middle of an illness, can mislead in either direction.

Start with a vitamin C (ascorbic acid) blood test.

Vitamin K: the functional test comes first

Vitamin K is genuinely part of the clotting story, because several clotting factors depend on it. But the honest framing matters here more than almost anywhere else on this page: dietary vitamin K deficiency is uncommon in healthy adults, and the clinically relevant assessment of vitamin K status for bleeding is functional, through PT/INR, rather than a direct measurement [16]. There is no single well-validated blood biomarker of vitamin K status, which is why status is usually inferred from function [17].

So a direct vitamin K measurement is a supporting test, not the first one. If bruising is the question, PT/INR answers the part that matters for bleeding, and a vitamin K level adds context to it.

One clarification, because the two forms are constantly conflated. Vitamin K1 (phylloquinone), from leafy greens, is the form that dominates the clotting conversation. Vitamin K2 (the menaquinones) is the form most of the supplement discussion is about, and it is usually discussed in the context of bone and cardiovascular health rather than bleeding. Start with a vitamin K blood test if you want the direct measurement, with PT/INR alongside it.

Bruising with bleeding elsewhere: check ferritin

This section needs a correction to a claim that circulates widely. Iron deficiency is not an established cause of easy bruising. Writing it as one gets the direction of the arrow backwards.

The real relationship runs the other way. Easy bruising alongside heavy menstrual periods, frequent nosebleeds, or bleeding gums is one of the patterns that raises suspicion of an underlying bleeding disorder [1][11], and when someone is losing blood repeatedly, iron deficiency is the downstream consequence of that blood loss. So ferritin is worth checking in that pattern not because low iron makes you bruise, but because it measures what the bleeding has cost you, and because fatigue and breathlessness from iron deficiency often arrive without being connected to the bleeding that caused them.

Where ferritin stops: it is an inflammatory marker as well as an iron marker, so inflammation, infection, and some liver and chronic conditions can hold it normal or push it high while iron stores are genuinely low [18]. A normal ferritin alongside an inflammatory condition is not a clearance, and the iron picture is finished with the wider iron studies rather than this one number.

If you would rather start with one broad panel

The tests above are the efficient route when you have a lead. If you do not have one, and the bruising sits alongside other symptoms you have not been able to place, one broad first pass can be more useful than guessing at a single marker.

That makes it a reasonable starting point when you have no specific lead at all, and a poor substitute when you do. The same limit applies as everywhere else here: a wider panel is a wider screen, not a wider set of verdicts.

When to see a clinician about easy bruising

Bloodwork sits alongside a clinical evaluation of bruising, not in place of one. Book an appointment if any of these apply:

  • the bruising is new, or noticeably different from how you have always bruised
  • it started after you began a new medicine or changed a dose
  • you have bruised easily your whole life, or close relatives do, and it has never been investigated
  • the bruising comes with heavy menstrual bleeding, frequent nosebleeds, or bleeding gums
  • your bloodwork came back normal and the bruising has not been explained

A clinician can do the things a blood panel cannot: look at the bruises and where they sit, examine the skin, take a structured bleeding history that covers dental work, surgery, childbirth, and family, and review every medicine and supplement you take [1]. In practice that conversation resolves more easy-bruising questions than any panel does.
