Pre-test considerations
A morning draw is preferred and fasting is recommended, since serum iron and transferrin saturation swing through the day and rise after iron-rich meals. If you take an iron supplement, the result reflects recent dosing, so keep timing consistent between tests and note your dose. Because ferritin climbs with any active inflammation, infection, or recent illness, an isolated high value is best interpreted alongside the rest of the iron studies.

What this test is for
Ferritin is the body's main iron-storage protein, and it is the most sensitive single marker of how much iron you have in reserve. Low ferritin is an early sign of iron depletion, often appearing well before anemia, and is commonly checked when investigating fatigue, hair loss, weakness, shortness of breath, restless legs, or poor exercise recovery. High ferritin reflects iron overload (as in hemochromatosis, where it can exceed 1000 ng/mL) but also rises with inflammation, liver disease, chronic infection, and autoimmune conditions, since ferritin is an acute-phase reactant. This panel pairs ferritin with the full iron studies (iron, TIBC, UIBC, transferrin saturation) and a complete blood count, so a low or high result can be read against your actual iron transport and red-cell picture rather than in isolation. Ferritin is less reliable late in pregnancy, when it falls even with adequate marrow iron.
