Thyroglobulin Antibodies (TgAb): What a High or Positive Result Means
Understand a high thyroglobulin antibody result, how it differs from TPO antibodies, and which TgAb test format fits your question.

A high or positive thyroglobulin antibody (TgAb) result means your immune system is making antibodies against thyroglobulin, a protein made by your thyroid. It is a sign of thyroid autoimmunity, most often Hashimoto’s thyroiditis. It is not a measure of how well your thyroid is working, and it is not a cancer test. What are thyroglobulin and thyroglobulin antibodies?
Thyroglobulin is a protein found in thyroid cells. A small amount normally leaks into the bloodstream. Sometimes the immune system makes antibodies against it, and the thyroglobulin antibody test measures those antibodies. MedlinePlus describes the antithyroglobulin antibody test.
You may see the test listed as TgAb, anti-Tg, anti-thyroglobulin antibody, or antithyroglobulin antibody. It is different from a thyroglobulin (Tg) test, which measures the protein itself.
Why is a thyroglobulin antibody test ordered?
TgAb is ordered for two different reasons:
- To look for autoimmune thyroid disease. Thyroid antibody tests are often ordered when TSH or thyroid hormone results are abnormal, to see whether an autoimmune condition is the cause. MedlinePlus covers when thyroid antibody tests are used.
- During thyroid cancer follow-up. After treatment for thyroid cancer, TgAb is measured because it can make the thyroglobulin result harder to measure accurately. MedlinePlus explains this use.
The two uses answer different questions. Most people searching for a high TgAb result are in the first group.
What does it mean if your thyroglobulin antibodies are high?
A positive result means antithyroglobulin antibodies were found in your blood. MedlinePlus lists these conditions in which they may be present:
- Hashimoto’s thyroiditis
- Graves’ disease or an overactive thyroid
- an underactive thyroid
- subacute thyroiditis
- type 1 diabetes
- systemic lupus erythematosus

A positive result is not a diagnosis on its own. Hashimoto’s can be present for years with no symptoms and normal thyroid hormone levels while antibodies are detectable in the blood. Should I worry about high thyroid antibodies?
High thyroid antibodies with normal TSH and free T4 do not require treatment with thyroid hormone, according to the American Thyroid Association. What matters over time is TSH, because some people with antibodies later develop low thyroid hormone levels. American Thyroid Association: Hashimoto’s thyroiditis. A high antibody result with an abnormal TSH, symptoms, a neck lump, or a pregnancy is a different situation and is covered below.
What is the difference between TPO antibodies and thyroglobulin antibodies?
Both are markers of thyroid autoimmunity, but they target different proteins. Thyroid peroxidase (TPO) antibodies target an enzyme the thyroid uses to make hormones. Thyroglobulin antibodies target the protein thyroid hormones are made from. MedlinePlus explains that these antibodies each target different proteins.
They are not equally informative:
- TPO antibodies are the stronger predictor of thyroid dysfunction. Reviews describe TgAb as less useful than TPO antibodies for predicting thyroid dysfunction, and identify finding people at higher risk of progressing to hypothyroidism as the most common use of TPO antibodies. - TgAb can be positive when TPO is not. The American Thyroid Association describes Hashimoto’s as an autoimmune disorder involving TPO and/or thyroglobulin antibodies, so either antibody can carry the autoimmune signal. American Thyroid Association.
- Isolated TgAb positivity carried measurable risk in pregnancy. A 2024 individual participant data meta-analysis of 25 pregnancy cohorts found that people positive for TgAb alone had a higher risk of subclinical and overt hypothyroidism than antibody-negative people. Their risk was lower than for TPO antibodies alone, and highest when both were positive. Osinga and colleagues, Thyroid, 2024.
- TgAb is the antibody that matters for thyroglobulin testing. TgAb can interfere with the thyroglobulin assay, which is why it is measured alongside thyroglobulin. Dwivedi and colleagues, 2023.
How are TPO, TgAb, and TSH read together?
Clinicians read the antibodies alongside thyroid function, not on their own. These are the common patterns and what each one points toward. They describe where the next question lies, not a diagnosis.

Which thyroglobulin antibody test should you order?
The right test depends on the question you are asking and the results you already have.
Quantity or presence result
Thyroglobulin antibody tests come in two result formats:
- Quantity: the lab reports a measured concentration next to its reference range. A number lets your clinician see how far above the lab’s limit a result sits.
- Presence: the lab reports whether antibodies are above its threshold, as positive or negative.

TgAb alone or with TPO antibodies
If you have never had thyroid antibodies checked, testing TPO and TgAb together answers the autoimmune question in one draw. Most people with Hashimoto’s have high levels of one or both. MedlinePlus. TgAb on its own fits a narrower question: you already have a negative TPO result and autoimmune thyroid disease is still being considered, since either antibody can carry the autoimmune signal.
If you already have a positive antibody result, you do not need to repeat it to track the condition. TSH and free T4 are what show whether thyroid function is changing. American Thyroid Association.
If you are in thyroid cancer follow-up, TgAb is ordered together with thyroglobulin by your cancer care team, as part of their monitoring plan rather than as a standalone screening test.
How are thyroglobulin antibody results reported?
Each lab sets its own reference range. A result flagged high is above that lab’s limit. Labs use different testing methods and ranges, so you can’t compare your result to a range found elsewhere, and you can’t compare results from different labs. There is no universal thyroglobulin antibody cut-off. Use the range printed on your own report.
What does a thyroglobulin antibody result of <1 mean?
A result written as a less-than value, such as <1, means the lab did not measure antibodies above that number. Read it against the reference range on the same report. A result below the lab’s upper limit is in its normal range, and a negative test result is a normal result. MedlinePlus.
What levels indicate Hashimoto’s?
No specific TgAb or TPO level diagnoses Hashimoto’s. The American Thyroid Association describes the diagnosis as positive TPO and/or thyroglobulin antibodies in a person with hypothyroidism. Hashimoto’s can also be identified when antibodies are high but thyroid hormone levels are still normal. The fact that a result is positive, read with TSH and free T4, carries more meaning than the number itself.
Do high thyroglobulin antibodies mean cancer?
No. A thyroglobulin antibody test is not a cancer test, and a positive result does not mean you have thyroid cancer. Blood tests generally do not help find thyroid cancer, and thyroid blood tests are usually normal even when a cancer is present. A thyroid lump is evaluated by neck examination, ultrasound, and, when needed, a fine needle biopsy. American Thyroid Association: thyroid cancer.
Research has looked at TgAb in people who already have thyroid nodules. Some studies found TgAb somewhat more often in people whose nodules turned out to be differentiated thyroid cancer. A 2018 review described that predictive value as weak and controversial, and not strong enough to use without other clinical findings. Jo and Lim, 2018.
Why is TgAb checked after thyroid cancer treatment?
After thyroidectomy and radioactive iodine for differentiated thyroid cancer, thyroglobulin levels usually become very low or undetectable, so a rising level is a warning sign for recurrence. TgAb can falsely lower or raise the thyroglobulin result, so cancer guidelines call for TgAb to be measured with every thyroglobulin test. That is why TgAb appears on cancer follow-up reports. It is checking whether the thyroglobulin number can be trusted, not looking for cancer itself.
Do thyroglobulin antibody levels change over time?
They can, but tracking them usually isn’t useful for autoimmune thyroid disease. The American Thyroid Association says following antibody levels over time does not help detect developing hypothyroidism or response to therapy, and that repeating antibody tests is not needed once Hashimoto’s is identified. After total thyroidectomy and radioactive iodine, antibodies fall gradually. In the studies a 2018 review summarized, TgAb took a median of about three years to disappear. Jo and Lim, 2018.
Thyroglobulin antibodies in pregnancy
Pregnant women may test positive for thyroglobulin antibodies. MedlinePlus. Thyroid antibody testing may be needed during pregnancy when you have thyroid disease or symptoms of it, because thyroid disease can affect both parent and fetus. MedlinePlus. In pooled pregnancy cohorts, TgAb positivity was linked with a higher risk of hypothyroidism during pregnancy, as described above. Osinga and colleagues, 2024.
When should you see a clinician or endocrinologist?
Take a positive TgAb result to your clinician, and ask about an endocrinologist, when it comes with an abnormal TSH or free T4, symptoms of an underactive or overactive thyroid, a lump or swelling in your neck, a current or planned pregnancy, or a history of thyroid cancer. Bring the original report with its units and reference range.
