Thyroid Peroxidase Antibodies: What High Results Mean
Understand what a positive TPO antibody result means, how laboratory ranges differ, and why TSH and symptoms matter.

High thyroid peroxidase (TPO) antibodies suggest that your immune system is reacting against your thyroid. How serious that is depends on your thyroid function, symptoms, and clinical history. A positive antibody result alone does not tell you whether your thyroid is underactive or establish a diagnosis. Mayo Clinic explains why the test is interpreted alongside other findings.
What are thyroid peroxidase antibodies?
Thyroid peroxidase is an enzyme your thyroid uses to make its hormones. TPO antibodies are immune-system proteins directed against that enzyme. The blood test measures those antibodies, not thyroid hormone levels. You may see it listed as TPOAb, anti-TPO, or thyroid peroxidase Ab. Mayo Clinic and MedlinePlus explain this distinction.
What does it mean if your thyroid peroxidase antibody is high?
TPO antibodies are the principal antibody marker of Hashimoto’s thyroiditis. They are found in most people with this autoimmune thyroid condition, but can also be positive in Graves’ disease. A positive result therefore cannot distinguish the two conditions by itself. Mayo Clinic Laboratories

How are TPO antibody results reported?
Your report usually gives a numerical antibody concentration, commonly in IU/mL, alongside a laboratory reference range or a positive/negative flag. A result marked high is above that laboratory’s reference limit. Mayo Clinic Laboratories’ TPO test illustrates this reporting format.
Reference ranges differ by laboratory and assay, so use the range printed on your own report. A cutoff you find online may belong to a different method. What levels indicate Hashimoto’s, and what is the normal range for TPO for Hashimoto’s?
There is no laboratory-independent TPO antibody threshold that diagnoses Hashimoto’s. The laboratory’s range establishes whether its assay calls your result elevated; the diagnosis requires clinical interpretation. There is no separate universal “normal range for Hashimoto’s.” Mayo Clinic
Can TPO antibodies be positive when TSH is normal?
Yes. TPO antibodies can be positive while thyroid function is still normal. Antibodies can precede overt hypothyroidism by years, and many people with positive antibodies never become hypothyroid. Thyroid-stimulating hormone (TSH) and free T4 help show how your thyroid is functioning now. A normal TSH does not erase antibody positivity; a positive antibody result does not mean your thyroid has already stopped making enough hormone. Does a higher titre mean more serious disease?
A higher TPO antibody titre, or level, is not a direct measure of thyroid damage or a severity score. For understanding your current health, the fact of positivity together with TSH, free T4, symptoms, and clinical context is more informative than the antibody number alone. The American Thyroid Association emphasizes thyroid-function monitoring rather than repeatedly tracking antibody levels.
Does high TPO antibodies mean cancer?
Elevated TPO antibodies indicate autoimmune thyroid activity and are not a cancer marker. This result neither diagnoses nor excludes thyroid cancer. Thyroid antibody testing and cancer evaluation answer different questions. Mayo Clinic’s TPO explanation
A thyroid lump or nodule is assessed separately, using clinical examination, ultrasound, and sometimes a needle biopsy. Even normal thyroid-function blood tests do not rule out cancer. How do you fix high thyroid peroxidase antibodies?
A positive TPO result does not automatically require treatment. The useful next step is to understand thyroid function and symptoms with your clinician, rather than pursue a lower antibody number as a goal in itself. American Thyroid Association
