T3 Uptake (T3RU) Test: What a Low or High Result Means

Understand T3 uptake, the thyroid binding test that is not a T3 level.

What is a T3 uptake test?

A T3 uptake test, also called T3 resin uptake (T3RU) or the thyroid hormone binding ratio, measures the proteins that carry thyroid hormone in your blood. It does not measure how much T3 you have. Its job is to help interpret a total T4 result, and sometimes a total T3 result, from the same blood draw. MedlinePlus explains the T3RU test.

The main carrier protein is thyroxine-binding globulin (TBG), which carries most of the T3 and T4 in the blood. T3 uptake estimates how many binding sites on those carrier proteins are still empty. ADLM describes what the test tells you.

Does a low T3 uptake mean low T3?

No. This is the most common misreading of the result. A low T3 uptake means the carrier proteins have more empty binding sites than expected. A high T3 uptake means they have fewer. Neither number is a level of the hormone T3. MedlinePlus describes what T3RU measures.

Why is T3 uptake still ordered?

T3 uptake is an older test. MedlinePlus notes that it is rarely used now that free T4 and TBG blood tests are available. MedlinePlus on how often T3RU is used. ADLM, the laboratory medicine society, goes further: it advises against ordering T3 uptake as a stand-alone test, says its only use is calculating a free thyroxine index, and recommends TSH and free T4 for checking for an underactive or overactive thyroid. It still appears on results because many standard thyroid panels report total T4, T3 uptake, and the free thyroxine index together. If your panel includes it, the useful question is not what the T3 uptake value says alone, but what the three results say together.

What is the free thyroxine index?

The free thyroxine index (FTI) is a number calculated from your total T4 and T3 uptake results. It estimates free T4, the unbound thyroxine available to your tissues, which a direct free T4 test measures outright. ADLM describes the free thyroxine index use.

The reason for combining them is that total T4 includes hormone bound to carrier proteins. When the amount of TBG changes, total T4 can change even though free T4 stays the same. The index puts the two together so a carrier-protein change is less likely to be mistaken for a thyroid problem. How do I read T3 uptake with total T4, the FTI, and TSH?

Read the results as a set. These patterns show how they usually relate. They are not diagnoses, and “high” and “low” mean outside the reference range printed on your own report.

TSH is the anchor. StatPearls notes that TSH is typically the first test used to assess for primary hypothyroidism, and that the characteristic findings are a high TSH with a low free T4. What causes a low T3 uptake?

A low T3 uptake usually reflects more carrier protein, or more empty binding sites. MedlinePlus lists:

  • Pregnancy.
  • Estrogen and birth control pills.
  • Other medicines, including antithyroid medicines, thiazide diuretics, opioids, and methadone.
  • Acute hepatitis.
  • Hypothyroidism.
  • An inherited condition with high TBG levels, in which thyroid function is usually normal.

What causes a high T3 uptake?

A high T3 uptake usually reflects less carrier protein, or fewer empty binding sites. MedlinePlus lists:

  • Hyperthyroidism.
  • Conditions that change blood protein levels, including nephrotic syndrome (kidney disease in which protein is lost in the urine), protein malnutrition, and chronic liver disease.
  • Kidney failure and serious illness.
  • Medicines, including anabolic steroids, heparin, phenytoin, warfarin, and high-dose salicylates.

What is a normal T3 uptake range?

T3 uptake is reported as a percentage. MedlinePlus gives 25% to 38% as a typical range and notes that ranges vary between labs, which use different methods. MedlinePlus on normal results. Use the range printed next to your result, and read a value outside it together with total T4, the free thyroxine index, and TSH rather than on its own.

Does low T3 uptake cause weight gain?

No. T3 uptake is a measurement of carrier proteins, not a hormone, so the number itself does not cause symptoms. Many low results come from more carrier protein, for example in pregnancy or with estrogen, while thyroid function stays normal. MedlinePlus on low T3RU causes.

Weight gain is listed among the early symptoms of hypothyroidism, though, and a low T3 uptake can be part of a hypothyroid pattern. MedlinePlus lists hypothyroidism symptoms. If you have weight gain with other symptoms such as fatigue, feeling cold, or constipation, TSH and free T4 are the tests that answer whether your thyroid is involved.

How do you fix a low T3 uptake?

There is no treatment aimed at the T3 uptake number. What matters is the reason behind it.

  • If the cause is more carrier protein, such as pregnancy, estrogen, or an inherited high TBG level, the result reflects that change rather than a thyroid problem. StatPearls on TBG changes.
  • If the full set of results points to hypothyroidism, a clinician confirms it with TSH and free T4. StatPearls notes that hypothyroidism can be treated with thyroid hormone. StatPearls on hypothyroidism.

Lifestyle changes are not a way to move this number, because it reflects proteins in your blood and your thyroid status, not a habit.

What does it mean if your T3 is low, and what T3 level is alarming?

These questions are about the hormone T3, which a T3 uptake test does not measure. A low T3 uptake is not a low T3. To check T3 itself, you need a total T3 or free T3 test, and there is no single T3 number that is alarming for everyone. Read it against your lab’s range and alongside TSH and free T4. When is a TSH and free T4 test the better choice?

If you are choosing a thyroid test rather than reading one you already have, TSH with free T4 is the more direct option. A panel with T3 uptake still makes sense when a clinician has asked for one, or when you want results in the same format as earlier panels so you can compare them over time.

When should I see a clinician about my T3 uptake?

Take your report to a clinician when the free thyroxine index or TSH is outside its range, when you have symptoms of an underactive or overactive thyroid, or when you are pregnant. Bring the full panel, including total T4, the FTI, and TSH, and a list of your medicines, because several of them change T3 uptake.
