Anion Gap Blood Test: What High and Low Results Mean

Understand the anion gap calculation, why normal ranges differ, and what to ask about a high or low result.

The anion gap is a number calculated from blood electrolyte results. Clinicians use it to help investigate acid-base disorders, especially metabolic acidosis. A high or low flag is a clue to interpret alongside the rest of your report, not a diagnosis on its own. MedlinePlus explains the test’s purpose and limitations.

Anion gap is not the albumin/globulin ratio (A/G ratio). Anion gap uses electrolytes; A/G ratio compares two groups of blood proteins. If your report says “anion gap,” a page about A/G ratio is answering a different question.

How is anion gap calculated?

The usual blood anion gap calculation subtracts chloride and bicarbonate from sodium:

Anion gap = sodium - (chloride + bicarbonate)

Some laboratories include potassium:

Anion gap = (sodium + potassium) - (chloride + bicarbonate)

Potassium raises the calculated result, so the formula must match the laboratory’s reference interval. These calculations estimate the difference between ions omitted from the formula; blood itself remains electrically neutral. They do not directly measure blood pH. On a metabolic panel, the bicarbonate component is commonly labeled CO2 or carbon dioxide. The gap is calculated from existing blood results rather than measured by a separate assay. Cleveland Clinic explains how the test works.

Results may use mEq/L or mmol/L. Their numerical values are equivalent for the singly charged ions in this calculation. Use values from the same blood sample, with consistent units. The CCJM review explains the units.

What is a normal anion gap?

Use the reference interval printed on your laboratory report. There is no universal normal or optimal anion gap. Laboratories use different measurement methods, and including potassium changes the calculation. Cleveland Clinic emphasizes laboratory-specific interpretation.

For context, published clinical references give different intervals:

  • StatPearls cites 4–12 mmol/L, while discussing the influence of albumin and phosphate. Source
  • A 2023 CCJM review describes 3–9 mEq/L with newer electrolyte assays, compared with a historical interval of 8–16 mEq/L. Source

These are examples from references, not interchangeable cutoffs for your report. Ask the testing laboratory which formula and interval it uses before comparing a result with a range found online.

What can a high anion gap mean?

A high anion gap can reflect a buildup of acids whose associated ions are not included in the calculation. In high-anion-gap metabolic acidosis, possible causes include:

  • Ketoacidosis: ketone accumulation, including in diabetes-related ketoacidosis or alcohol-related illness.
  • Lactic acidosis: lactate accumulation, such as during shock or impaired oxygen delivery.
  • Kidney failure: reduced acid excretion allows acids to accumulate.
  • Certain poisonings: methanol, ethylene glycol, or salicylates such as an aspirin overdose.

These are clinical possibilities, not diagnoses that a high flag can distinguish. Merck Manual describes these causes.

Metabolic acidosis can also occur with a normal anion gap. For example, bicarbonate loss from diarrhea or renal tubular disorders can be accompanied by increased chloride. A gap within range therefore does not rule out an acid-base problem. Merck Manual explains normal-gap acidosis.

What can a low anion gap mean?

An unexpected low anion gap often needs confirmation before interpretation. The CCJM review identifies measurement error as the most common cause and recommends repeating electrolyte sampling to confirm the finding. CCJM’s practical approach

Low albumin can lower the gap

Albumin carries negative charge and contributes to the usual gap. Less albumin can mean a lower gap. Causes of low albumin include inflammation, impaired liver production, malnutrition, or protein loss through the kidneys or gut. CCJM explains the albumin relationship.

Low albumin can also hide an elevated anion gap. A clinician may account for albumin when assessing possible acidosis, even if the reported gap looks normal. StatPearls discusses this limitation.

Other causes need clinical context

Less common explanations include excess lithium and increased positively charged blood proteins, including some monoclonal proteins. Certain substances can interfere with chloride measurement and produce an artificially low result. A persistent low gap can justify investigation; it does not diagnose a protein disorder or poisoning. CCJM reviews these causes and testing pitfalls.

A low gap alone does not establish alkalosis or explain a symptom. It is an electrolyte calculation, not a direct acidity measurement. Albumin, the other results, and your clinical history matter. StatPearls explains what the gap represents.

What should you do with a flagged result?

Discuss the complete report with the clinician who ordered it. Bring the laboratory’s reference interval, previous results, your medication list, and any symptoms. Ask whether an unexpected result needs confirmation and whether other testing would help explain it. A flag alone does not prove that treatment is needed. Cleveland Clinic describes this follow-up.

Seek medical care if you develop symptoms of metabolic acidosis, such as rapid breathing, confusion, or unusual lethargy. Do not wait for routine repeat testing when you are seriously unwell. MedlinePlus describes the symptoms and when to seek medical help.
