How this calculator works
The clinical PhenoAge formula, developed by Levine and colleagues using NHANES data, combines nine blood measurements with your age at the draw. All ten inputs are required. The calculation runs in your browser.
For adults aged 20 or older. Use exact values from one draw and the units on your report. A lower score does not establish that an intervention extends life.
Method implementation checked October 28, 2026. Educational information, not a diagnosis or a treatment recommendation.
What does your biological age mean?
PhenoAge translates nine routine blood measurements and your calendar age into an age-equivalent estimate of mortality risk. It was developed from population data, so it describes how your combination of results compares with patterns in that population. The useful next step is to understand the measurements behind the score and how they change over time. [1]
Which blood results shape your PhenoAge?
The formula combines markers from a complete blood count, a metabolic panel and a CRP test. They contribute different information to one score. A change in the estimate can therefore come from several small changes or a larger change in one input. [1]
- Blood cells: white blood cell count, lymphocyte percentage, MCV and RDW.
- Blood chemistry: albumin, creatinine, glucose and alkaline phosphatase.
- Inflammation: CRP, entered as an exact positive result in the correct unit.
How should you read a younger or older estimate?
A PhenoAge below your calendar age corresponds to a lower modeled mortality risk than the age-based reference. A higher estimate corresponds to a higher modeled risk. Neither tells you how old each organ is or how many years you have left. [3]
- Keep both numbers: your age at the draw and the PhenoAge estimate.
- Review values flagged on the original report, even if the overall estimate is younger.
- Compare results from the same clock. A difference between clocks is not a change over time.
What can make the score change between tests?
Blood results can reflect temporary circumstances as well as longer-term patterns. CRP, for example, can rise with an infection or injury. Because CRP is an input to PhenoAge, a change during an illness can move the estimate without showing a lasting change in how you are aging. [2]
- Keep the draw date and note whether you were unwell.
- Use one complete draw for each calculation, rather than mixing dates.
- Check which inputs changed before attributing a score change to a new habit.
Why might another age test give a different answer?
Clinical PhenoAge uses routine blood measurements. DNA methylation clocks, including DNAm PhenoAge, use patterns in DNA methylation. They measure different signals and are not interchangeable. Tracking one method consistently is more informative than treating two different clocks as before-and-after results. [1]
What a fuller panel adds
Explore biological-age testing with Mito- These nine markers complete PhenoAge. Broader testing adds information beyond this score, rather than extra inputs to the calculator.
- Blood sugar and nutrients: HbA1c, iron studies, folate and vitamin B12 add measurements the PhenoAge formula does not use.
- Kidney and liver chemistry: urine albumin and creatinine, plus AST, ALT, bilirubin and urea provide more results to review alongside this estimate.
- Blood pressure and BMI add context beyond bloodwork, when available in your health record.
- Mito’s member biological age combines LinAge2 with age-adjusted body-system assessments and clinician review. It is a separate assessment from this PhenoAge estimate.

Frequently asked questions
Can I calculate biological age without blood tests?
No. You need all nine blood measurements and your age at the draw.
Can I combine results from different dates?
Use results from the same blood draw. Combining dates creates an input set that does not describe one point in time, making the result harder to interpret.
What if my CRP result says “less than” a number?
That is a reporting limit, not an exact value. Do not enter zero or substitute the limit as though it were measured. Use an exact positive numeric result in the units printed on your report.
Is this the biological age shown in my Mito account?
This tool uses PhenoAge. Mito’s member assessment uses LinAge2 and combines it with age-adjusted, clinician-reviewed body-system findings. The dashboard number uses a different model and more health information.
