Estradiol vs LH: Key Differences Explained

Estradiol and luteinizing hormone (LH) time ovulation together. LH is made by the pituitary gland. Estradiol is made by the growing follicle in the ovary. For most of the cycle, estradiol keeps LH low. Late in the follicular phase, high and rising estradiol makes the pituitary more responsive, which helps set off the brief mid-cycle LH surge that releases an egg. Reading the two together tells you more about where a cycle is than either one alone.
What Is Estradiol?
Estradiol (E2) is the main estrogen in women of childbearing age who are not pregnant. In women it is made mainly by the ovaries, and it rises and falls across the menstrual cycle. Men make smaller amounts, largely from testosterone. Results are usually reported in picograms per milliliter (pg/mL).
What Is LH?
Luteinizing hormone (LH) is made by the pituitary gland, under direction from the hypothalamus in the brain. In women who have periods, LH helps control the menstrual cycle, and a rise in LH about two weeks after a period usually means ovulation is happening. In men, LH tells the testicles to make testosterone, and LH levels normally change very little.
How Estradiol and LH Interact
Estradiol has two effects on LH, depending on timing.
- Negative feedback: for most of the cycle, estradiol and progesterone from the ovary signal the pituitary to keep LH secretion in check.
- Positive feedback: late in the follicular phase, estradiol from the dominant follicle is the main signal that makes the pituitary more sensitive, which helps produce the mid-cycle LH surge.
After ovulation, the follicle becomes the corpus luteum and makes progesterone as well as estradiol, and both hormones then hold LH down again until the next cycle.

Reading Estradiol and LH Together
- High estradiol with an LH surge around mid-cycle: this fits normal ovulation. A high estradiol value on its own, drawn near ovulation, is expected rather than excessive.
- Low estradiol with low LH: the signal from the brain may be low. Lower than normal LH is often linked to a problem with the pituitary gland or hypothalamus, and in women with missed periods, low LH and FSH can also be linked to extreme exercise, stress, malnutrition, or being very underweight.
- Low estradiol with high LH: the ovaries are responding less to a strong pituitary signal. In women aged 45 or older, high LH can mean menopause is approaching.

Is a single LH blood test enough to confirm ovulation? No. The LH surge is brief and can be missed by one draw. A progesterone test in the luteal phase is used to check whether ovulation happened.

Why is LH tested with FSH? Both come from the pituitary and respond to the same brain signals, so they are usually measured together along with estradiol.

Do home ovulation tests measure the same thing? They measure LH in urine to spot the surge. They do not measure estradiol.

Why do men get LH tested? In men, LH drives testosterone production, so it helps show whether a low testosterone result starts in the testicles or in the pituitary.
