Pre-test considerations
No fasting required. Because estradiol changes across the menstrual cycle, timing matters: note the day of your cycle, since results are interpreted against cycle phase. If you take estrogen, oral contraceptives, or other hormone therapy, mention it, as these affect the result. This standard immunoassay is calibrated for reproductive-range levels; for postmenopausal, male, or aromatase-inhibitor monitoring, the ultrasensitive LC/MS version is preferred.

What this test is for
Estradiol (E2) is the most potent estrogen during the reproductive years, produced mainly by the ovaries and responsible for regulating the menstrual cycle and maintaining female reproductive tissues. Levels shift predictably across the cycle, so estradiol is read alongside LH and FSH to pinpoint cycle phase, evaluate irregular or absent periods, and monitor ovulation induction and IVF stimulation. Low estradiol with low or normal LH/FSH points toward hypothalamic or pituitary causes (including stress, low body weight, or overexercise), while low estradiol with high FSH points toward primary ovarian failure or the menopausal transition. Normal or high estradiol with irregular cycles can accompany PCOS or estrogen-producing tumors.
This is the standard serum estradiol by immunoassay, well suited to the higher levels seen in cycling women and fertility treatment. For the very low levels found in postmenopausal women, men, or anyone on aromatase inhibitors, the Estradiol Ultrasensitive (LC/MS) test is the more accurate choice, and the LH, FSH, and Estradiol panel bundles the gonadotropins when you want the full cycle picture in one draw.
