  • Track a number linked to how long you live

    Fitness is one of the strongest predictors of lifespan. In a study of more than 120,000 adults, having below-average rather than above-average fitness raised the risk of death about as much as smoking or diabetes.

Not the right test if

  • An injury stops you from running or cycling hard

    The result depends on pushing to your limit, so wait until you can.

Do I need a referral?

No. You order online, and a physician's order is arranged for you when one is needed. Some people need a short letter from their own doctor, explained in the next answer.

Will I need a letter from my doctor?

Only if two or more items on the safety screen apply to you, and your age alone can count as one. The others are symptoms when you exert yourself, smoking, a diagnosis of high blood pressure, diabetes, high cholesterol or a heart or lung condition, a joint problem made worse by exercise, and a close relative with early heart disease. The one-minute check tells you before you pay. If you need a letter, a short note from your own doctor is enough. We'll tell you how to send it, it is reviewed within a few business hours, your order doesn't expire, and there's no fee while approval is pending.

Does insurance cover it?

Rarely. Fitness tests are elective, so plan to pay out of pocket.

What the VO2 Max Test measures

VO2 max is the most oxygen your body can take in and use during all-out exercise, measured in milliliters per kilogram of body weight per minute. In the lab you wear a mask that captures your breath while the effort steadily climbs, and a metabolic cart measures how much oxygen you use. A graded exercise test with breath analysis is the gold standard way to measure it.

Cardiorespiratory fitness is one of the strongest predictors of how long you live. The American Heart Association recommends treating it as a vital sign, checked regularly like blood pressure. Unlike most vital signs, you can move it with training.

The same test finds your aerobic and anaerobic thresholds and your real maximum heart rate. That gives you training zones built on your own body instead of an age formula. There are no needles and no radiation.

What your VO2 Max Test report shows

Your VO2 max is compared with people of your age and sex. Retesting on the same type of machine tells you more than any single reading.

VO2 max

The most oxygen your body used at peak effort, in mL/kg/min. The headline fitness number, and the one to track over the years.

Aerobic threshold

Your oxygen use and heart rate at the point where easy, steady effort ends. That heart rate is the top of your zone 2.

Anaerobic threshold

Your oxygen use and heart rate at the point where effort becomes hard to sustain. It sets the floor for tempo and interval work.

Max heart rate

Your highest heart rate, measured at your peak instead of estimated from 220 minus your age, so your zones fit you.

Heart rate recovery

How far your heart rate falls 1 and 2 minutes after you stop. A faster drop generally goes with better fitness.

METs

Your effort in multiples of your resting energy use, from the start of the test to your peak. In a large review of 33 studies, each extra MET of fitness was linked with about 13% lower risk of death.

What is a good VO2 max for your age?

Treadmill VO2 max in mL/kg/min for healthy US adults. The median is the middle value for your age and sex: half of people score above it.

What this test can't tell you

  • It isn't a cardiac stress test, so it can't diagnose or rule out heart disease.
  • Your result depends on effort. Stopping early gives a lower number than your true max.
  • Treadmill and bike results differ, so retest on the same type of machine.
  • Changes of less than about 5% between tests are within normal day-to-day variation.

How it works

What to expect from your VO2 Max Test

  1. 1 Check eligibility and order

    Answer the one-minute safety check, then check out on Mito. If you need a clearance letter, you'll know before you pay.

  2. 2 Book your time

    Choose a partner center and your preferred times. You get a confirmation, usually within hours.

  3. 3 Do the test

    Staff fit a mask and a heart rate strap, you warm up, then the speed, incline or resistance rises every few minutes. After about 8 to 15 minutes of steadily harder effort you stop when you can't continue, then cool down while your recovery is tracked. Treadmill or bike depends on the center. Block 1 to 1.5 hours.

  4. 4 Get your report

    Results usually arrive within 24 hours and land in your Mito dashboard, next to your labs.

  5. 5 Train and retest

    Set your zones from your thresholds. Retest after 8 to 12 weeks of training, or once a year, on the same type of machine.

What does a VO2 max test measure?
Your VO2 max, your aerobic and anaerobic thresholds with the heart rate at each, your maximum heart rate, your heart rate recovery and your METs from start to peak.
How long does a VO2 max test take?
The hard part takes about 8 to 15 minutes, after a short warm-up and before a cool-down. Block 1 to 1.5 hours for the whole visit.
How can I improve my VO2 max?
Mostly with steady zone 2 training plus one or two harder interval sessions a week. Beginners often gain 10 to 15% in a few months, which is why the test is worth repeating after 8 to 12 weeks.
How do I prepare for a VO2 max test?
Don't exercise for 24 hours before. Come hydrated and well fed, but avoid a heavy meal in the 2 to 3 hours before. Wear exercise clothes and shoes. Block 1 to 1.5 hours for the visit.
What is a good VO2 max for my age?

How accurate is Apple Watch or Garmin VO2 max?
Watches estimate VO2 max from heart rate and pace. In a 2025 study, Apple Watch underestimated it by about 6 mL/kg/min on average, an error of about 13%. Consumer devices are commonly off by 5 to 15%. A lab test measures it directly.
How much does a VO2 max test cost?
Prices range from about $80 at some university labs to $500. Most fitness and sports clinics charge $150 to $250. Your price is shown on this page, and members pay less. It includes booking, any physician's order the center needs, and the report in your Mito dashboard, and it is the same whichever partner center you choose.
Is a VO2 max test safe?
For most healthy adults, yes. Serious problems are rare, about 1 to 5 per 10,000 exercise tests in large studies, which include many people with heart disease. Staff supervise the test and you can stop at any time. The safety screen flags higher risk before you book.
How often should I retest my VO2 max?
After 8 to 12 weeks of training to see progress, or once a year to track your fitness over time. A change of more than about 5% is real.
Can I test my VO2 max at home?
Not directly. Watches and fitness apps give estimates. Measuring VO2 max takes a mask and gas analysis during a maximal effort, which is what the lab test does.
Where can I get a VO2 max test near me?
At select partner centers. When you book, you choose a center and your preferred times, and the center confirms, usually within hours.
Is a VO2 max test worth it?
It is most useful if you want a baseline for a number tied to longevity, want training zones built on your own thresholds, or want to know whether your watch is close. If you don't plan to change your training, one test still gives you a baseline to return to.
How does pricing work?

Every test shows the member price next to the standard non-member price, so you can see what membership saves you. The member price is our cost, covering the lab and what it takes to run the service. It is never a profit on the test itself. Mito makes its money on the membership, not on marking up your tests. Membership is $9 per month or $99 per year, and any applicable lab order or draw fee is itemized before you pay.

Yes. Mito's lab tests are HSA/FSA eligible, so you can pay with pre-tax dollars.

Who can order a scan or fitness test?

You must be 18 or older to order a scan or fitness test through Mito. Before you add one to your cart, you answer a few short health questions so we can check that the test is right for you.

  • Coronary Artery Calcium Scan: for people aged 40 to 75 with at least one heart risk factor and no known heart disease or heart symptoms. Risk factors include high cholesterol or blood pressure, diabetes, a family history of heart disease, a BMI over 25, smoking, or an inactive lifestyle. You'll also give your primary care provider's details.
  • DEXA scans: not available if you're pregnant or may be pregnant, or if you've recently had a barium exam. If you have metal implants, a physician reviews your answers first.
  • VO2 Max Test: not available if you're pregnant. Depending on your answers, you may need clearance from your own doctor before your appointment can be confirmed.

Do I need a doctor's order for a scan?

No. If your test needs a physician's order, it's arranged for you as part of booking. You don't need to contact your own doctor or bring an order with you.

The one exception is the VO2 Max Test. If your health questionnaire shows you need medical clearance, we'll ask your own doctor to sign off first.

How do I book my scan appointment?

After you buy your scan, open your Mito dashboard and:

  1. Answer the testing center's health questions, and add your emergency contact and primary care physician details.
  2. Enter your ZIP code to find the nearest testing center.
  3. Pick two appointment times: your preferred time and a backup.

The center then confirms one of those times, usually within 48 hours. We'll email you at each step: when we receive your booking request, when the center confirms your appointment, and a reminder before your visit. If your appointment is cancelled or missed, we'll let you know by email too.

What's the difference between "appointment requested" and "appointment confirmed"?

Requested means we've sent your preferred times to the testing center, but the appointment isn't final yet. It may still be waiting on the center, on your health questions, or on a doctor's clearance.

Confirmed means the center has finalized your appointment and expects you at the date and time in your confirmation email.

Please don't treat a requested appointment as final. If it hasn't been confirmed within 48 hours, message Mito Concierge or email help@mitohealth.com.

What happens if I cancel or reschedule my scan appointment?

You can reschedule or cancel from your Mito dashboard.

  • Rescheduling: your first two reschedules are free when you make them more than 24 hours before your confirmed appointment. Each reschedule after that costs $25. If the center changes your time, that doesn't count toward your free reschedules.
  • Cancelling more than 24 hours ahead: your purchase stays open while our care team reviews whether to refund it or use it to rebook. Refunds aren't automatic, so message us if you have a preference.
  • Cancelling 24 hours or less before your appointment, or missing it: your purchase is forfeited. A new appointment needs a new purchase at the full price.
How do I prepare for my scan or fitness test?

Follow the instructions in your confirmation email, because they can affect your results. If you don't prepare as instructed, the center may not be able to do the test.

For every appointment

  • Bring a photo ID and your confirmation or reminder email.
  • Arrive about 5 minutes before your appointment. Many centers have little waiting space, so don't arrive much earlier. If you're late, the center may not be able to see you.
  • Don't bring children, and avoid bringing guests.

DEXA scans

  • Wear comfortable clothes, such as gym clothes, with no metal or hard plastic. That means no zippers, buttons, underwire, jewelry, or watches.
  • Don't eat for 2 to 4 hours before. Drink water normally.
  • Avoid intense exercise, saunas, and steam rooms for 12 hours before.
  • Tell the staff if you have metal in your body, such as a pin or a joint replacement.

VO2 Max Test

  • Don't exercise for 24 hours before.
  • Be well hydrated and fed, but avoid a heavy meal in the 2 to 3 hours before. A light snack is fine.
  • Wear the clothes and shoes you normally exercise in.
  • Allow 1 to 1.5 hours.

Coronary Artery Calcium Scan

  • No alcohol or caffeine for 4 hours before.
  • No strenuous exercise for 24 hours before.
  • Wear clothes that are easy to take off, with no metal. A gown may be provided.
I'm having a DEXA scan and a VO2 max test on the same day. How should I prepare?

Eat a light meal about 4 hours before your appointment, then drink only water. Your DEXA scan will be done first, with the VO2 max test right after. Avoid exercise for 24 hours before your appointment.

Why might I need my doctor's clearance for a VO2 max test?

A VO2 max test is a hard exercise test. If your health questionnaire shows you need medical clearance first, we'll contact you with a Physician Assessment form.

Have your own doctor complete and sign it, then send it back to help@mitohealth.com. Once the signed form is received and reviewed, your appointment can be confirmed.

When and how will I get my scan results?

Your results are uploaded to your Mito dashboard, where you can view and download them. They usually arrive within 2 business days of your appointment. If you haven't seen them after 5 business days, message Mito Concierge or email help@mitohealth.com and we'll check.

Can I talk to someone about my scan results?

Yes. Consultations are available to review your scan results and talk through what they mean for you. You can book one from your Mito dashboard.
