Quick summary

There is no single PSA number that is normal for every man or that diagnoses prostate cancer. In one published study of men aged 40 to 79 without evidence of prostate cancer, the upper age-band reference values for total PSA rose from 2.5 ng/mL in the 40s to 6.5 ng/mL in the 70s. These are examples from that study, not universal clinical cutoffs. Compare your result with the reference interval on your own lab report first. [1, 2]

Total PSA levels by age

The table shows the 95th-percentile upper values reported in a 1993 community-based study of 471 men in Minnesota who had no evidence of prostate cancer on the study’s evaluation. The study used one PSA assay and population, so its values may differ from another laboratory’s interval or a clinician’s interpretation. They are not a way to decide on screening or rule cancer in or out. [1, 2]

AgePublished upper reference value for total PSA
40 to 49 years2.5 ng/mL
50 to 59 years3.5 ng/mL
60 to 69 years4.5 ng/mL
70 to 79 years6.5 ng/mL

A 70-year-old man may therefore see 6.5 ng/mL quoted as an age-specific upper reference value, but it is not a personal target or a guarantee that a lower result is harmless. This study did not establish an age band for men 80 or older. NCI notes that laboratories can use different ranges and that no single threshold separates a normal from an abnormal result. Use the interval printed on your report ahead of an online chart. [1, 2]

Can you have a high PSA and no cancer?

Yes. A high total PSA result can occur without prostate cancer. Benign prostatic hyperplasia (BPH, an enlarged prostate) and prostatitis (prostate inflammation) can raise PSA. A result above a published age reference or your lab’s interval does not diagnose cancer, and a result within either interval does not rule it out. A higher result generally raises concern for cancer, but even a very high result cannot be explained from the number alone. [2]

If a result is unexpectedly high, the circumstances around the blood draw matter. Recent ejaculation or vigorous cycling can raise PSA temporarily. Prostate infection or inflammation, a recent prostate biopsy, a bladder procedure, or a recently placed urinary catheter can also affect the result. Tell your clinician about these factors when reviewing the result and deciding on the screening question or any follow-up. [2, 3]

What can change a PSA reading?

FactorPossible effect on measured total PSA
BPH or prostatitisCan raise PSA without cancer. [2]
Recent ejaculation or vigorous cyclingCan raise PSA temporarily. [2]
Recent prostate biopsy or inflammationCan keep PSA raised while the prostate recovers. [2]
Recent bladder procedure or urinary catheterCan raise PSA without cancer. [3]
Finasteride or dutasterideCan lower the measured PSA, changing how a clinician interprets it. [2]

Do not adjust the number yourself if you take finasteride or dutasteride. Make sure the clinician interpreting your result knows about the medication. [2]

Total PSA, free PSA, and the next question

This chart is about total PSA, the amount reported by a total PSA blood test. Free PSA is a different measurement that can add context to a total result. See the free PSA percentage reference for that narrower question. For test pricing, use the separate PSA test cost guide.

Mito lists total PSA with reflex to free PSA and free and total PSA. The product pages describe the current tests. A result still needs interpretation in your own clinical context.
