Biotin and Blood Tests: How Supplement Interference Happens

Biotin can interfere with some laboratory tests and produce a result that is falsely high or falsely low. The risk depends on the dose, timing, assay design, and laboratory platform. Tell the ordering clinician and laboratory about every biotin-containing supplement before the draw.
Do not use a universal online rule for when to stop biotin. The appropriate instruction must match the supplement exposure and the exact assay.
What is biotin?
Biotin is vitamin B7. It can appear in multivitamins, prenatal vitamins, B-complex products, and supplements marketed for hair, skin, or nails. The amount in a product can be much higher than the daily amount supplied by food, so report the product and dose rather than saying only that you take a vitamin.
The FDA has warned that biotin above recommended daily intake can interfere with certain in vitro diagnostic tests.

How does biotin interfere with a blood test?
Some laboratory assays use the strong binding between biotin and streptavidin as part of the method that detects a target substance. Free biotin from a person’s sample can disrupt that system. The result can move either upward or downward depending on how the assay is designed.
This is analytical interference. It does not necessarily mean the true level in the body changed. It also does not affect every laboratory test or every manufacturer’s version of a test.

Which blood tests can biotin affect?
The FDA specifically highlights certain assays that use biotin technology, including some hormone tests and tests for cardiac markers such as troponin. Which individual result is vulnerable depends on the assay platform used by the laboratory. A test name alone is not enough to determine the risk.
This means a generic list can mislead in both directions. It may imply that every thyroid, hormone, or cardiac test is affected, or it may omit a locally used assay that is sensitive to biotin. Ask the laboratory whether the ordered method has known biotin interference.
Can biotin cause a false thyroid result?
Some thyroid immunoassays use biotin-streptavidin technology and can be affected, but the direction of error depends on the method. A pattern that appears internally inconsistent or does not fit the clinical picture should prompt review of supplements and the assay rather than an automatic diagnosis.
How long should you stop biotin before a blood test?
There is no single stop interval that is safe to apply to every product and assay. The answer can depend on the dose, how recently it was taken, kidney function, the laboratory method, and the urgency of testing. Ask the ordering clinician or laboratory for the instruction attached to the exact test.
Do not stop a prescribed product, prenatal vitamin, or clinician-directed high-dose biotin without checking with the prescriber. If testing is urgent, do not delay care merely to complete a supplement washout. Tell the care team about the exposure so they can choose an appropriate method or interpret the result with that limitation.
What to tell the laboratory
Before the draw, report:
- the product name and label dose
- when you took the last dose
- how often you take it
- whether it is a multivitamin, prenatal vitamin, hair or nail supplement, or prescribed therapy
- which tests are being ordered
- whether a prior result seemed inconsistent with symptoms or other measurements
Bring the bottle or a clear photo of the Supplement Facts label when the dose is uncertain.
What if blood was already collected?
Tell the ordering clinician and laboratory as soon as you realize that biotin exposure was not reported. Do not interpret a suspicious result by simply reversing its direction. The laboratory may need to identify the assay, add a limitation, use a different method, or repeat the collection.
The FDA has reported concern about falsely low results in some biotin-sensitive troponin assays. Chest pain or other possible emergency symptoms require urgent evaluation regardless of supplement use or a prior laboratory result.
Biotin is not the same as fasting
Fasting controls recent food and beverage intake. Biotin interference is a method-specific issue that can remain relevant even when a person completed the food fast correctly.
