Low T4 Symptoms and What the Result Means

A low T4 result can occur with an underactive thyroid, but its meaning depends on TSH, whether the test measured free or total T4, medicines, pregnancy, illness, and the laboratory method. Possible symptoms include fatigue, cold intolerance, constipation, dry skin, slowed thinking, muscle aches, and weight change. These symptoms have many possible causes.

Why Free and Total T4 Are Different
Most T4 circulates bound to proteins. Total T4 measures bound and unbound hormone, while free T4 estimates the unbound fraction. Changes in binding proteins can change total T4 without producing the same change in free T4. Calculated measures such as the free thyroxine index are different again.
What the Pattern Can Show
Low free T4 with high TSH can fit a different pattern from low free T4 with a low or normal TSH. A clinician may consider the full thyroid pattern, symptoms, medicines, pregnancy status, and whether acute illness could affect testing. One result cannot diagnose the cause.
