Why measure instead of using a calculator?

The best formula lands within 10% of measured for about 4 in 5 people without obesity and about 7 in 10 with obesity. Everyone else is off by more, often 150 calories a day or more, and a calculator can't tell you which group you're in. The Academy of Nutrition and Dietetics prefers a measured RMR over formulas when one is available.

What the Resting Metabolic Rate (RMR) Test measures

Your resting metabolic rate is the calories your body burns in a day just to stay alive: breathing, pumping blood, running your organs and keeping warm. For most people it is 60 to 75% of everything they burn in a day. It is the floor every calorie target is built on.

The test uses indirect calorimetry. While you rest, a metabolic cart measures the oxygen you breathe in and the carbon dioxide you breathe out. Burning calories always uses oxygen at a known rate, so your oxygen use converts directly into calories. The balance of the two gases also shows whether you're burning mostly fat or carbohydrate.

Online calculators guess your RMR from your sex, age, height and weight. They can't see how much muscle you carry, and about a quarter of the difference in resting burn between people isn't explained by body size, age or body composition at all. Two people with the same height and weight can burn very different amounts. This test measures yours.

There's no single normal RMR. It depends on your size and, most of all, your muscle, which is why your number sits next to a prediction for someone like you.

Measured RMR (calories per day)

The calories your body burns in 24 hours at complete rest, measured from your breath. Add activity and digestion on top to get your total daily needs.

Predicted RMR and how yours compares

The number a standard prediction equation expects for someone of your sex, age and size. If you measure 1,200 and the prediction is 1,400, you burn about 14% less at rest than calculators assume, roughly 200 calories a day.

RER: fuel use at rest

The ratio of carbon dioxide out to oxygen in. About 0.7 means mostly fat, about 0.85 a mix, and about 1.0 mostly carbohydrate. At rest it reflects your recent meals, not a fixed "fat burner" type.

The 10% margin is the accuracy cutoff used in a systematic review of RMR prediction equations (Frankenfield and colleagues, 2005). A measured to predicted ratio below 0.90 is listed as an indicator of low energy availability in the 2023 International Olympic Committee consensus statement on Relative Energy Deficiency in Sport.

What this test can't tell you

  • It's one morning's snapshot. Sleep, stress, illness and your menstrual cycle can shift it, so retest under the same conditions. A change needs to be more than about 6 to 8% to be real.
  • It isn't a thyroid test or a diagnosis of any condition. Thyroid function is checked with a TSH blood test.
  • Your resting fuel use reflects your recent meals and the fast, not a fixed metabolic type.
  • It covers rest only. Calories burned through activity and digestion are still estimates.

What is an RMR test?
A resting metabolic rate test measures how many calories your body burns at rest. You breathe normally into a mask or hood while a metabolic cart measures the oxygen you use, which converts into calories per day.
What does an RMR test show?
Your measured calories burned at rest per day, the number a prediction equation expects for someone your size, and your RER, which shows whether you burn mostly fat or carbohydrate at rest.
How do I prepare for an RMR test?
Book a morning appointment so the fast is easy. For 12 hours before, have nothing but water: no food, caffeine or alcohol. Wait at least 4 hours after nicotine, appetite suppressants or allergy medication, as they can affect your result. Skip moderate or vigorous exercise for 24 hours. Leave extra time so the trip in is calm, without brisk walking, running or lots of stairs. Wear comfortable clothes. Food, caffeine and exertion raise your burn for hours, which is why the rules are strict. Have breakfast and coffee straight after.
How long does an RMR test take?
You breathe into the mask or hood for about 10 to 20 minutes. Allow 30 to 60 minutes for the whole visit.
How accurate is an RMR test?
Repeat tests under the same conditions usually agree within 2 to 4%. A change between two tests needs to be more than about 6 to 8%, roughly 100 to 135 calories a day, to be a real change rather than normal day-to-day variation.
What's the difference between RMR and BMR?
BMR is the strict lab version, measured on waking after an overnight stay in a temperature-controlled room. RMR is the practical version and runs slightly higher, often by up to about 10%. Most calculators use the two terms interchangeably. Your total daily burn is RMR plus digestion and activity.
What is a good resting metabolic rate?
There's no single good number, because it depends on your size and muscle. What matters is how yours compares with a prediction for you. For a 45-year-old woman who is 165 cm and 75 kg, the standard formula predicts about 1,395 calories a day. A measured 1,256 to 1,535 is in line; a measured 1,200 would be about 86% of predicted.
Can an RMR test tell me if I have a thyroid problem?
No. Resting metabolism was once used to check the thyroid, but blood tests replaced it in the 1950s. Today thyroid function is checked with TSH and thyroid hormone blood tests. A low RMR can be a reason to check your labs, not a diagnosis.
Is an RMR from a DEXA scan accurate?
Some DEXA reports list an RMR, but a DEXA scanner can't measure your breathing, so that figure is calculated from your lean mass. Lean mass explains about 63% of the differences in resting burn between people, so it beats a height and weight formula. It is still an estimate: about a quarter of the difference isn't explained by body composition. An RMR test measures it.
How often should I get an RMR test?
There's no official schedule. Most people test once for a baseline, then again after losing 5 to 10% of their body weight, or every 3 to 6 months during an active diet or GLP-1 course. Test at the same time of day with the same preparation each time.
How much does an RMR test cost?
University labs charge about $40 to $100, and commercial clinics usually $110 to $240. Your price is shown on this page, and members pay less. It includes booking and the report in your Mito dashboard, and it is the same whichever partner center you choose.
Is an RMR test worth it?
It is most useful when your calorie number matters: weight loss has stalled, you're on a GLP-1, you train hard, or you're in midlife and your body is changing. If your weight and habits are stable and working, a calculator may be close enough.
