What this test is for
Zinc transporter 8 (ZnT8) is a protein in the insulin-producing beta cells of the pancreas, and antibodies against it are a marker of the autoimmune process behind type 1 diabetes. A positive ZnT8 antibody result points to autoimmune (type 1) diabetes rather than type 2, which matters because adults presenting with new diabetes are misclassified roughly 40% of the time, and latent autoimmune diabetes in adults (LADA) is easy to mistake for type 2. It is also used to assess presymptomatic risk in people with a family history of type 1 diabetes or a high-risk HLA genotype. ZnT8 adds the most value alongside the other islet autoantibodies (GAD65, IA-2, and insulin autoantibodies); guidelines recommend testing all four, since staging early type 1 diabetes requires at least two positive antibodies. On its own a single antibody narrows the question rather than settling it.

Where do I get tested?
