TSH receptor antibodies (TRAb) are autoantibodies that bind the thyroid's TSH receptor, where they can either stimulate or block it. They are the key marker for diagnosing Graves' disease, the most common cause of hyperthyroidism, when symptoms like rapid heartbeat, weight loss, heat intolerance, tremor, anxiety, and bulging eyes point to an overactive thyroid. A positive result confirms the autoimmune cause and separates Graves' from other reasons for a low TSH, which changes how the condition is treated. TRAb is also used to assess relapse risk before stopping antithyroid medication and to gauge risk of thyroid dysfunction in pregnancy, since the antibodies cross the placenta and can affect the newborn. Because it reflects both stimulating and blocking antibodies, it is read alongside TSH, free T4, and free T3.
