Pre-test considerations
No fasting required. Timing is what matters: to capture anaphylaxis, the sample should be drawn within about 30 minutes to 6 hours of the suspected event, then compared against a baseline level drawn at least 24 hours after all symptoms have fully resolved. A single resting measurement is fine for assessing basal tryptase.

What this test is for
Tryptase is a serine protease stored in mast cells and released both steadily at baseline and in larger amounts when mast cells activate. A persistently elevated basal level points to mast cell burden or hyperactivity: the most common cause is hereditary alpha tryptasemia, but it is also seen in mastocytosis and other myeloid disorders, allergic disease, chronic kidney failure, and some chronic infections. A transient spike above your own baseline is one of the few objective signs that an acute reaction was true anaphylaxis. The serum test detects total tryptase and does not separate the alpha and beta forms, so a single elevated number is interpreted with your history and, when relevant, a repeat draw.
