Pre-test considerations
No fasting required. The accuracy depends on collecting all urine over the full 24 hours: discard the first morning void, then collect every void for the next 24 hours including the next morning's first. Keep the container cool during collection and avoid strenuous exercise beforehand, since heavy exertion can transiently raise urine protein. The creatinine result helps flag an under- or over-collected sample.

This test measures total protein excreted in urine over 24 hours, with creatinine used to confirm the collection was complete and to express the protein/creatinine ratio. Healthy kidneys spill almost no protein, so a rising amount points to impaired filtration from diabetes, hypertension, glomerular disease, nephrotic syndrome, or drug-related kidney injury. The 24-hour collection captures total daily protein loss and is the more thorough way to quantify and stage proteinuria when a spot sample has already flagged a concern. If your question is specifically early diabetic or hypertensive kidney damage, the albumin (microalbumin) and creatinine collection targets the smaller albumin leak that appears first.
