Pre-test considerations
No fasting is required for total protein. Prolonged tourniquet time and dehydration can falsely raise the result, so normal hydration helps. If you have had recent IV fluids, the value can read low from dilution.

What this test is for
Total protein measures all the protein circulating in serum, mainly albumin plus the globulins. Albumin keeps fluid inside blood vessels and carries hormones and vitamins, while globulins include antibodies and transport proteins, so the combined number reflects nutrition, liver synthesis, kidney protein loss, and immune activity. Low total protein points to liver disease, malnutrition, malabsorption, or protein loss through the kidneys or gut; high total protein reflects dehydration, chronic inflammation, or excess globulin production seen with chronic infection or some blood cancers. Because albumin and globulins move in opposite directions in many conditions, the number is most useful alongside an albumin and albumin/globulin breakdown.
This is the serum measurement, which assesses protein status in the blood. The separate 24-hour and random urine total protein tests measure protein lost into urine for kidney evaluation, and the Total Protein and Albumin Panel adds the albumin and globulin split that explains an abnormal total.
