What this test is for
Tissue transglutaminase (tTG) antibodies are the main blood markers for celiac disease, an immune reaction to gluten that inflames and flattens the lining of the small intestine and drives malabsorption, fatigue, bloating, diarrhea, weight changes, anemia, and the skin condition dermatitis herpetiformis. This test measures the IgG class of tTG antibodies.
The IgA version (tTG IgA) is the standard first-line screen for most people. The tTG IgG test is the right choice when someone is IgA-deficient, a condition more common in celiac disease, because in that situation the IgA result can be falsely negative and IgG carries the diagnostic signal. Accurate results depend on eating gluten regularly beforehand: antibody levels fall and can normalize once gluten is removed, so testing after starting a gluten-free diet can miss the disease. A positive result is followed by intestinal biopsy for confirmation.
