Pre-test considerations
No fasting required. Keep eating a normal gluten-containing diet until the blood is drawn, since cutting gluten lowers antibody levels and can produce a false negative. If you have already gone gluten-free, the result may understate disease, so discuss timing first.

What this test is for
Tissue transglutaminase IgA (tTG IgA) is the antibody the immune system produces against an enzyme in the gut lining when gluten triggers inflammation in celiac disease. It is the single best blood test for screening celiac disease and dermatitis herpetiformis, with high sensitivity in people eating a normal gluten-containing diet. A high level points strongly toward celiac disease and warrants confirmation, usually with an intestinal biopsy; antibody levels fall over time once gluten is removed, so the test also tracks whether a gluten-free diet is working. One important caveat: people with selective IgA deficiency can have a falsely low result, which is why total IgA or the IgG-based antibodies (deamidated gliadin or tTG IgG) are checked when IgA is low.
This is the IgA tissue transglutaminase test, the recommended starting point for most people. The separate tTG IgG and deamidated gliadin IgG tests are used mainly when IgA deficiency is present, and the broader celiac panels add those antibodies for harder diagnostic cases.
