Pre-test considerations
No fasting required. Thyroglobulin antibodies interfere with the measurement, so this test is interpreted together with antibody status. For surveillance, use the same lab and method each time, since thyroglobulin values are not always comparable across assays.

What this test is for
Thyroglobulin is a protein made only by thyroid tissue, which makes it a useful tumor marker rather than a general thyroid-function test. Its main role is monitoring people treated for well-differentiated thyroid cancer: after the thyroid is removed and any remnant ablated, a rising thyroglobulin signals residual or recurrent disease, including metastasis. Thyroglobulin antibodies are measured alongside it because they are common and they distort the result, typically pushing thyroglobulin falsely low in the immunometric assays used here, so interpreting one without the other is unreliable. Low thyroglobulin synthesis also appears in infants with goitrous hypothyroidism. This is a monitoring and surveillance test, not a way to diagnose thyroid cancer or evaluate everyday thyroid symptoms.
