Pre-test considerations
Collect in the morning, ideally before 10 a.m., when testosterone peaks. A low or borderline result should be confirmed with a second morning sample. Acute illness, poor sleep, and recent intense exercise can temporarily lower levels. If you use testosterone or anabolic steroids, note the dose and timing, since they raise the result.

What this test is for
Total testosterone measures all the testosterone circulating in your blood, both the fraction bound to proteins and the small free portion that acts on tissues. It is the primary screening test for low testosterone in men, commonly checked when investigating low libido, fatigue, depressed mood, loss of muscle mass, erectile difficulty, or poor exercise recovery, and in women when evaluating PCOS, irregular cycles, acne, or excess hair growth. Levels follow a daily rhythm and peak in the morning, so a borderline or low result is best confirmed with a repeat morning draw before acting on it.
This is the total measure and the right default for a first look. Because 60 to 90 percent of testosterone is bound to SHBG and biologically unavailable, conditions that shift SHBG (aging, thyroid disease, obesity, diabetes) can move total testosterone without changing what reaches your tissues; when total is borderline, the separate Free or Bioavailable Testosterone tests clarify the active fraction.
