Reflex testing
If the treponemal antibody screen comes back positive, confirmatory follow-up testing (a nontreponemal RPR, reflexing to a titer, and a second treponemal test when results disagree) runs automatically to distinguish active infection from past exposure and to stage it. This is done at no additional cost.
Pre-test considerations
No fasting or special preparation is required, and timing during the day does not matter. Antibodies take a few weeks to develop after exposure, so testing very early after a suspected contact can miss a recent infection; retesting later may be warranted.

What this test is for
This test detects antibodies to Treponema pallidum, the bacterium that causes syphilis, using a treponemal immunoassay. It is the starting point of the reverse screening algorithm: a positive result reflects either an active or a previously treated infection, since treponemal antibodies usually persist for life after exposure. That is why a reactive result needs confirmation and staging with a nontreponemal test before it means active infection. Syphilis often produces subtle or intermittent signs (a painless sore, a rash on the palms and soles, or nothing at all), so antibody screening is the reliable way to catch it. It is commonly used for routine STI screening, evaluation after a possible exposure, and prenatal care.

Where do I get tested?
Choose an available partner lab at checkout. If your cart contains multiple blood tests, Mito helps consolidate the eligible tests at one lab when possible. At-home kits and scans follow the collection or appointment process shown on their product page.

When will I get my results?
