Smooth muscle antibodies (SMA) are autoantibodies directed against components of smooth muscle, and they are a central marker in the workup of autoimmune liver disease. They are found in roughly 85% of people with autoimmune (chronic active) hepatitis and in a smaller share of those with primary biliary cirrhosis, which makes SMA useful when liver enzymes are elevated and the cause is unclear. They are usually negative in viral hepatitis, drug-induced liver injury, and extrahepatic biliary obstruction, so a positive result helps point toward an autoimmune cause rather than those alternatives. A positive screen supports the diagnosis alongside other findings such as ANA, elevated IgG, and liver biopsy, rather than confirming it alone.
