What this test is for
PAI-1 (plasminogen activator inhibitor-1), encoded by the SERPINE1 gene, is the body's main brake on fibrinolysis, the process that dissolves blood clots. This test reads the 4G/5G variant in the gene's promoter: the 4G allele is associated with higher circulating PAI-1, which slows clot breakdown and is linked to a higher risk of arterial and venous thrombosis, myocardial infarction, progression of atherosclerosis, and recurrent pregnancy loss. The 4G/4G genotype carries the strongest association, 5G/5G the lowest, and 4G/5G sits in between. Because this is a fixed genetic result, a single test settles your genotype for life; it describes inherited tendency, not a clot in progress, so it is interpreted alongside personal and family clotting history and other thrombophilia markers.
