Pre-test considerations
No fasting required, and timing during the day does not materially affect the result. If you recently started or changed an iron, B12, or folate supplement, note when, since the reticulocyte response is exactly what this test can capture. Recent transfusion can alter the picture.

What this test is for
Reticulocytes are young red blood cells, released from the bone marrow before they fully mature, so counting them shows how actively your marrow is producing red cells right now. A high count means the marrow is ramping up production, typically in response to blood loss, hemolysis, or recovery after starting iron, B12, or folate. A low count points to underproduction, as seen in iron, B12, or folate deficiency, marrow suppression, or aplastic anemia. This is the marker that separates the two main causes of anemia: not enough production versus loss or destruction of cells. It is most useful read alongside a CBC and iron studies, which is why those markers are reported with it here.
