Pre-test considerations
Anticoagulants distort this test, so results are most reliable off blood thinners. Warfarin and other vitamin K antagonists lower Protein C activity, while heparin and direct oral anticoagulants (dabigatran, argatroban, rivaroxaban, apixaban, edoxaban) can falsely raise it. Testing is typically deferred until well after an acute clot and after stopping interfering drugs, under physician guidance. No fasting required.

What this test is for
Protein C is a vitamin K-dependent protein that helps keep clotting in check by breaking down activated clotting factors. This functional assay measures Protein C activity, not just its quantity, so it reflects how well the protein actually works. Low activity points to deficiency, which can be inherited or acquired, and is associated with venous thrombosis including deep vein clots and pulmonary embolism. Acquired deficiency shows up with warfarin and other vitamin K antagonists, vitamin K deficiency, liver disease, malignancy, and DIC. It is commonly checked during a thrombophilia workup after an unexplained or early clot, or a strong family history of clotting.
