Pre-test considerations
No fasting required. Do not test within six weeks of a prostate biopsy, and avoid testing immediately after cystoscopy, as both substantially raise PSA. Finasteride and dutasteride lower PSA by roughly 50 percent, and antibiotics for prostatitis can drop it by about 30 percent, so note any current medications. Ejaculation and a digital rectal exam have small, variable effects. For ongoing monitoring, stay with one assay method, since results from different methods are not interchangeable at these low levels.

What this test is for
PSA is a glycoprotein made by prostate tissue, and its level in blood rises with prostate cancer, benign enlargement (BPH), prostatitis, and recent prostate trauma. This ultrasensitive version measures PSA down to a detection limit at or below 0.01 ng/mL, far lower than standard assays, which makes it the right test for tracking men after surgery or radiation for prostate cancer, where the question is whether PSA is climbing back from near zero.
This is the high-precision monitoring form, not the routine screening test. The separate total PSA (2.5 ng/mL cutoff) test is the standard screening marker, and the free and total PSA test helps refine the decision to biopsy when total PSA is mildly elevated. Because assay methods are not interchangeable at these low concentrations, serial monitoring should stay on one consistent method for results to be comparable over time.

Where do I get tested?
