Pre-test considerations
No fasting required. Avoid ejaculation for 24 to 48 hours before the draw and schedule the test before, not after, a digital rectal exam or prostate procedure, since both can transiently raise PSA. Vigorous cycling shortly before the test can also elevate the result.

Prostate-specific antigen (PSA) is a glycoprotein made by prostate cells and released into the blood, where the total level reflects the combined activity of normal, enlarged, and cancerous prostate tissue. Elevated PSA is linked to prostate cancer, benign prostatic hyperplasia (BPH), and prostatitis, and it can also rise after ejaculation, vigorous cycling, or a recent prostate exam. This version reports total PSA against a 2.5 ng/mL cutoff, a lower threshold than the traditional 4.0 ng/mL, which flags borderline results sooner and is useful for younger men or for tighter year-over-year tracking.
