Pre-test considerations
No fasting required, but timing and handling matter. Prolactin is highest in sleep and rises with stress, exercise, eating, and even the needle stick, so the draw is best done between 8 and 10 a.m., and some clinicians place the line and rest 30 minutes before drawing. Avoid breast or nipple stimulation beforehand. Note any medications, since several (including some antipsychotics, antidepressants, and antinausea drugs) raise prolactin.

What this test is for
Prolactin is the pituitary hormone behind milk production, and elevated levels cause irregular or absent periods, infertility, low libido, erectile dysfunction, unexplained breast milk production, and sometimes headaches or vision changes from a growing pituitary tumor. This version measures prolactin both directly and after a 1:100 dilution to rule out the high-dose hook effect, a quirk of immunoassays where extremely high prolactin paradoxically reads as normal or only mildly raised. That matters when imaging shows a large pituitary mass but the standard prolactin is lower than expected: the diluted result distinguishes a true prolactin-secreting macroadenoma (prolactinoma) from a large nonfunctioning adenoma, which changes whether medication or surgery is the next step. For a first look at prolactin with typical symptoms, the standard Prolactin test is the right starting point; this dilution protocol is for the specific case where a hook effect is suspected, and the separate Total and Monomeric test is used to check for macroprolactin when a high reading needs explaining.
