Pre-test considerations
Prolactin rises with stress, exercise, sleep, eating, and breast or chest stimulation. Draw the sample in the morning, ideally 3 to 4 hours after waking, after sitting calmly for 15 to 30 minutes, and avoid breast stimulation and vigorous exercise beforehand. Note any medications, since many antidepressants and antipsychotics elevate prolactin.

What this test is for
Prolactin is a hormone made by the pituitary gland, best known for driving milk production but also influencing menstrual cycles, ovulation, and testosterone. Persistently high levels outside of pregnancy and breastfeeding (hyperprolactinemia) commonly cause irregular or absent periods, galactorrhea, and infertility in women, and low libido, erectile dysfunction, and low testosterone in men. The usual culprits are a prolactin-secreting pituitary adenoma, hypothyroidism, kidney disease, chest wall or breast stimulation, and a long list of medications including many antidepressants and antipsychotics. Because exercise, stress, sleep, and a recent meal can all push prolactin up transiently, a single mildly elevated result is often rechecked under controlled conditions before it means anything.
This is the standard total prolactin measurement. If a result is high, the Total and Monomeric form sorts out whether it reflects biologically active prolactin or inactive macroprolactin, and the Amenorrhea Panel pairs prolactin with FSH and LH when the question is why periods have stopped.
