Pre-test considerations
No fasting is required. PTH follows a daily rhythm, so a consistent morning draw makes results more comparable when you are tracking over time. A simultaneous calcium measurement is needed to interpret the result. The intact assay is not affected by biotin or most supplements.

What this test is for
Parathyroid hormone (PTH) is released by the parathyroid glands to keep blood calcium in a tight range, pulling calcium from bone, retaining it in the kidneys, and activating vitamin D. The intact assay measures the whole, biologically active molecule, so it is not thrown off by inactive fragments that build up in kidney failure. High or inappropriately normal PTH alongside high calcium points to hyperparathyroidism; low PTH with low calcium points to hypoparathyroidism; suppressed PTH with high calcium raises the question of a calcium-raising tumor. It is also a core monitoring marker in chronic kidney disease and after bariatric surgery, where calcium and vitamin D deficiency can drive PTH up.
PTH is only interpretable next to a calcium level measured at the same time.
