Pre-test considerations
No fasting required. Collect a solid stool sample; very watery or diarrheal samples can dilute the result and lower the reading. You can continue pancreatic enzyme replacement therapy, since it does not affect this measurement. Keep the sample refrigerated or frozen until you return it.

Pancreatic elastase-1 is an enzyme made by the pancreas and passed largely intact into stool, so its concentration in a stool sample reflects how much digestive enzyme the pancreas is producing. Low levels indicate exocrine pancreatic insufficiency, where the pancreas does not make enough enzymes to digest food properly. It is commonly checked when investigating chronic diarrhea, greasy or foul-smelling stools that float, bloating, unexplained weight loss, or nutrient deficiencies, and it is relevant in chronic pancreatitis, cystic fibrosis, and after pancreatic surgery. Because it is measured directly in stool, you can stay on pancreatic enzyme replacement therapy during the test without distorting the result. A clearly low result usually leads to imaging or further pancreatic workup to find the cause.
