What this test is for
Oxcarbazepine (Trileptal) is an anticonvulsant used for partial and generalized tonic-clonic seizures, and after a dose it is rapidly converted to 10-hydroxycarbazepine, the metabolite that does the work. This test measures that active metabolite in serum, because it has a much longer half-life than the parent drug and is what blood-level monitoring is based on. Effective seizure control is generally seen with levels between roughly 15 and 35 mcg/mL, while levels above 35 mcg/mL are associated with toxic effects such as dizziness, drowsiness, double vision, unsteadiness, and nausea. Monitoring is most useful when adjusting the dose, when seizures break through, when toxicity is suspected, or when another medication, pregnancy, or kidney function changes how the drug is cleared. The metabolite is excreted almost entirely by the kidneys, so falling clearance can push levels up.
