Thiamine (vitamin B1) is an essential nutrient your body cannot make, used as a coenzyme for the reactions that turn carbohydrates, fats, and alcohol into usable energy, and for nerve and neurotransmitter function. Whole blood is the preferred sample because thiamine pyrophosphate, the main active form, concentrates inside red cells and accounts for about 90% of blood thiamine. Early deficiency is nonspecific: fatigue, irritability, poor memory, sleep disturbance, low appetite, and abdominal discomfort. Severe or prolonged deficiency causes beriberi (heart failure or peripheral neuropathy) and Wernicke-Korsakoff syndrome, with confusion, eye-movement problems, and unsteady gait.
