Pre-test considerations
No fasting required. For occupational exposure monitoring, the recommended draw is at the end of the shift at the end of the work week. Otherwise timing is flexible, though levels reflect recent exposure, so a recent large fish meal can raise the result.

What this test is for
Whole blood mercury measures recent and ongoing exposure to mercury, the heavy metal most people encounter through eating fish and shellfish. Blood is the preferred sample for organic mercury (methylmercury), which concentrates in red blood cells and predominantly affects the central nervous system; symptoms of significant exposure include tremor, impaired coordination and speech, constricted visual fields, numbness, and irritability. High consumption of large predatory fish (tuna, swordfish, shark, king mackerel) is the most common nonindustrial source, and CDC data found roughly 6% of childbearing-age women at or above the reference level, which is why pregnancy planning is a common reason to check it.
This whole blood test is the right choice for suspected organic mercury (methylmercury) exposure. Inorganic and metallic mercury exposure, which is mostly occupational and affects the kidneys and lungs, is tracked better with a urine mercury test, and the separate serum and red-blood-cell mercury forms are used for specific clinical questions.
