Pre-test considerations
No fasting required. Timing matters more than diet: smears are most informative when drawn around fever spikes, and a single negative result does not exclude infection, so repeat sampling over 24 to 48 hours is common. Note recent travel (country and dates) and any anti-malarial medication taken, since both affect interpretation.

What this test is for
This test examines a stained blood smear under the microscope to find and identify parasites circulating in the blood, most importantly malaria (Plasmodium) and Babesia. It is the standard way to confirm these infections and to estimate percent parasitemia, the fraction of red cells infected, which guides treatment decisions and tracks the response to therapy. Malaria is the main concern after travel to endemic regions and typically presents with fever, chills, sweats, headache, and body aches; Babesia is tick-borne and can cause fever, fatigue, and hemolytic anemia. A single negative smear does not rule out infection, since parasites cycle in and out of the bloodstream, so repeat smears over 24 to 48 hours are often needed when suspicion is high.
