Pre-test considerations
No fasting required, and timing relative to meals does not matter. Timing relative to infection does: antibodies are often undetectable in the first 14 days after exposure, so testing very early can miss a real infection and may need to be repeated. Because antibodies can persist for months to years after an infection resolves, a positive result is interpreted alongside your symptoms and exposure history.

What this test is for
Lyme disease is a tickborne infection caused by the bacterium Borrelia burgdorferi, spread mainly by blacklegged ticks in the upper midwestern, northeastern, and mid-Atlantic regions. This test looks for IgG and IgM antibodies your immune system makes against Borrelia, using the modified two-tier testing algorithm that first runs a combined total-antibody screen and, if positive or equivocal, confirms with separate IgG and IgM assays. It is designed to support diagnosis in people with symptoms consistent with Lyme, such as an expanding rash, fever, fatigue, joint pain, or facial palsy, plus plausible tick exposure. Timing matters: antibodies can be undetectable in the first two weeks of infection, and once present they can linger for months to years after the infection has cleared, so results are read against your symptoms, exposure history, and how long ago they started.
