Pre-test considerations
No fasting required. Tell the lab you are taking warfarin (Coumadin), since this version is built to interpret lupus anticoagulant testing during anticoagulation. Other anticoagulants such as heparin or direct oral anticoagulants also affect clot-based assays, so note all blood thinners and their timing. The specimen requires multiple frozen citrate plasma tubes, so it is drawn and processed at the lab.

What this test is for
This panel screens for antiphospholipid syndrome (APS), an autoimmune condition in which the immune system makes antibodies that promote abnormal clotting. It combines the two pillars of diagnosis: clot-based lupus anticoagulant testing (dRVVT with hexagonal phase confirmation, plus PT, INR, and thrombin time) and the solid-phase antibodies (cardiolipin IgG and IgM, beta-2 glycoprotein I IgG and IgM). APS is a leading cause of unexplained venous or arterial thrombosis, recurrent miscarriage, and stroke at a young age, and it often appears alongside lupus and other autoimmune disease. This version is configured for patients already taking warfarin (Coumadin), which interferes with standard clot-based lupus anticoagulant assays, so the panel includes confirmation steps and clotting-time markers to interpret results despite anticoagulation.
A positive antiphospholipid result needs to be confirmed on a repeat sample at least 12 weeks later, since transient antibodies can appear after infection. Persistent positivity, paired with a clotting or pregnancy event, is what defines the syndrome.
