Pre-test considerations
Timing is the critical variable. Draw a trough level, just before your next dose and at least 6 to 12 hours after the last one, so results are comparable visit to visit. Keep your dosing schedule and the draw time consistent. Fasting is not required. The sample must not be collected in a lithium heparin tube, which falsely raises the result.

Lithium is a mood stabilizer used mainly for bipolar disorder, and it has a narrow window between an effective dose and a toxic one, which is why blood levels are monitored directly. This test measures the concentration of lithium in serum to confirm you are in the therapeutic range and not drifting toward toxicity. Levels that run too low risk inadequate symptom control, while levels that run too high cause tremor, nausea, vomiting, diarrhea, lethargy, slurred speech, and in severe cases confusion, rigidity, and seizures. Because sodium status, hydration, kidney function, and interacting medications all shift lithium levels, the result is read alongside how you feel and your recent dose. Levels are checked frequently when starting or adjusting a dose, then periodically once stable.
