What this test is for
Lp-PLA2 is an enzyme produced by macrophages and inflammatory cells inside the artery wall, where it acts on oxidized LDL and generates pro-inflammatory products that drive plaque formation. Measuring its activity reflects inflammation specifically in the vessel wall, which makes it useful for refining coronary heart disease risk in people with no prior cardiac event. Unlike hs-CRP, which rises with infection, injury, and any systemic inflammation, Lp-PLA2 is not elevated by general inflammatory conditions, so it is a more vascular-specific signal and shows relatively small biological variation.
This measures enzyme activity rather than the broad lipoprotein particles its catalog siblings cover. Lp(a) captures inherited particle risk, and the NMR, ion-mobility, and ultracentrifugation panels count and size LDL and HDL particles; Lp-PLA2 instead reads the inflammatory activity layered on top of that lipid burden, which is why it is often paired with a standard lipid or particle assessment rather than used alone.
