Pre-test considerations
Fast for eight hours before the draw; water is fine. The triglyceride result is the most sensitive to recent food and alcohol, so avoid both the night before. Note that the NMR particle analysis is only added (at additional charge) when LDL cholesterol is under 130 mg/dL, or when triglycerides exceed 800 mg/dL via a direct-LDL step first.

What this test is for
This is a conventional lipid panel (total cholesterol, HDL, triglycerides, calculated LDL, non-HDL, and the LDL/HDL ratio) with a built-in reflex: if LDL cholesterol comes back under 130 mg/dL, it automatically adds lipoprotein particle analysis by NMR to count LDL particle number (LDL-P). This matters because cholesterol content and particle number often disagree. In the MESA study, people whose LDL-C looked normal but whose LDL-P was high carried the higher cardiovascular risk, and risk tracked with particle number, not the cholesterol estimate. The NMR reflex also yields a lipoprotein insulin-resistance score, a qualitative read on insulin resistance and diabetes risk that a basic panel cannot provide.

Non-HDL cholesterol is total cholesterol minus HDL, which leaves every artery-clogging particle behind: LDL, VLDL, and their remnants. It correlates closely with ApoB and often predicts cardiovascular risk better than LDL alone, especially when triglycerides run high or LDL looks deceptively normal.
