Pre-test considerations
No fasting required. In menstruating women, timing is critical: these hormones shift across the cycle, and they are often drawn early in the cycle (around day 3) for baseline assessment, so note your cycle day. Hormonal contraceptives, fertility medications, and hormone therapy alter all three results. If you take any of these, note them, since interpretation depends on it.

This panel measures three hormones that work together to run the reproductive axis: LH and FSH, signaling hormones released by the pituitary, and estradiol (E2), the main estrogen produced by the ovaries. Reading them together is more informative than any one alone, because the pattern distinguishes where a problem sits: high FSH and LH with low estradiol points to the ovaries (perimenopause, menopause, or diminished ovarian reserve), while low FSH and LH point upstream to the pituitary or hypothalamus. It is commonly used to investigate irregular or absent periods, fertility questions, low libido, hot flashes, and to confirm menopausal status. In men, LH and FSH alongside estradiol help clarify the cause of low testosterone and distinguish testicular from pituitary causes. Results depend heavily on cycle timing, so the day of collection matters for interpretation.

In men, estradiol is made from testosterone and, when elevated, can drive gynecomastia, low libido, or mood changes, so it's often checked alongside testosterone when those symptoms show up or when starting hormone therapy.

FSH is the pituitary signal that drives egg maturation in the ovaries and sperm production in the testes. In women, rising FSH reflects declining ovarian reserve, making it central to fertility planning, cycle irregularity workups, and confirming perimenopause or menopause. In men, elevated FSH points to impaired sperm production, while low levels in either sex suggest the pituitary or hypothalamus isn't sending the signal at all, useful context alongside LH and estradiol.

LH is the pituitary signal that triggers ovulation in women and drives testosterone production in the testes of men, acting as the messenger between brain and gonads. In women, tracking LH alongside FSH and estradiol clarifies where you are in your cycle, confirms ovulation timing for fertility planning, and helps investigate irregular periods or suspected PCOS. Where do I get tested?
