Pre-test considerations
No fasting required. This is not a blood draw: it requires a respiratory specimen (sputum, bronchial or transtracheal wash, lung or respiratory tissue, or pleural fluid) collected in a clinical setting and submitted in a sterile leak-proof container, kept refrigerated. Collecting the sample before or early in antibiotic treatment improves the chance of detection.

What this test is for
Legionella pneumophila is a waterborne bacterium that causes Legionnaires' disease, a severe pneumonia, and Pontiac fever, a milder flu-like illness. This test uses direct immunofluorescence to detect Legionella antigen in a respiratory specimen such as sputum, bronchial or transtracheal wash, lung tissue, or pleural fluid, confirming the organism's presence at the site of infection. It is ordered when pneumonia is severe, not responding to standard antibiotics, or linked to a possible outbreak or recent travel, where identifying Legionella changes antibiotic choice. A negative result does not fully exclude infection, since antigen detection depends on specimen quality and the species or serogroup present, so it is read alongside culture and urine antigen testing.
