Pre-test considerations
No fasting required. Hemolysis falsely elevates LD, so a clean draw matters; prolonged contact of serum with red cells and freezing both invalidate the sample. Timing matters for cardiac questions: LD isoenzyme changes typically become informative 36 to 55 hours after symptom onset.

What this test is for
Lactate dehydrogenase (LD) is an enzyme present in almost every cell, released into the blood when cells are damaged. This panel uses electrophoresis to separate total LD into five isoenzyme fractions, each concentrated in different tissues: LD1 and LD2 in heart, red blood cells, and kidney; LD3 in lung; LD4 and LD5 in liver, skeletal muscle, and white blood cells. The pattern is what matters: an LD1:LD2 "flip" (LD1 greater than LD2) points to heart muscle, red cell, or kidney injury and shows up in myocardial infarction, hemolytic anemia, and megaloblastic anemia, while an LD5 rise points to liver disease or skeletal muscle damage. A high total LD with no single fraction elevated (the isomorphic pattern) is seen with widespread processes such as malignancy, inflammation, or uremia.
