Pre-test considerations
Overnight fasting is required, typically 8 to 12 hours, water only. Schedule a morning draw for a true fasting value. Hemolyzed samples are rejected, so a clean venipuncture matters. If you use insulin or glucose-lowering medication, note the dose and timing, since these affect the result.

What this test is for
Insulin is the hormone that moves glucose out of the blood and into cells, and it also regulates fat metabolism. This test measures intact insulin by LC/MS/MS, a highly specific method that quantifies the insulin molecule itself. High fasting insulin points to insulin resistance, metabolic syndrome, or, in the setting of low blood sugar, causes of hyperinsulinemia such as insulinoma, sulfonylurea effect, or exogenous insulin. Low insulin alongside high glucose reflects reduced pancreatic output.
The LC/MS/MS intact assay is the specialized form, used when precision matters, such as separating insulin made by the body from injected insulin or evaluating unexplained hypoglycemia; the standard immunoassay insulin test is the routine option for most metabolic questions, and pairing intact insulin with C-peptide sharpens the picture further.
