Pre-test considerations
No fasting required. This test should be done before starting insulin therapy: once insulin injections begin, the body produces antibodies to injected insulin that this assay cannot distinguish from autoimmune ones, making the result uninformative.

What this test is for
Insulin autoantibodies (IAA) are antibodies the immune system makes against its own insulin, and they are one of four islet autoantibodies that mark the autoimmune process behind type 1 diabetes (T1D). A positive result points to autoimmune beta-cell destruction and helps distinguish T1D from type 2 diabetes and from latent autoimmune diabetes in adults (LADA), a distinction that is misclassified in roughly 40% of adults presenting with new T1D. IAA is usually the first autoantibody to appear in young children and becomes less common with age, which makes it most informative early. It is not useful once insulin treatment has started, because injected insulin itself triggers antibodies that the test cannot tell apart from autoimmune ones. Because diagnosing presymptomatic T1D requires at least two positive islet autoantibodies, IAA is interpreted alongside GAD65, ZnT8, and IA-2 rather than on its own.
