What this test is for
HPA-1 is a genetically determined antigen on the surface of platelets, and people fall into one of three genotypes depending on which versions they inherit. The clinical concern is alloimmunization: when someone lacking the common HPA-1a antigen is exposed to it (through pregnancy or transfusion), the immune system can make antibodies that destroy platelets. This is the main genetic cause of neonatal alloimmune thrombocytopenia, where maternal antibodies cross the placenta and attack the fetus's platelets, and it also drives post-transfusion purpura. Genotyping identifies whether a parent carries the at-risk combination, which guides monitoring and management in current or future pregnancies. Because it reflects inherited DNA, the result is fixed for life and does not need repeating.
